If you are NOT trending to goal, it is time to review your facility's Root Cause Analysis (RCA) and make rapid cycle improvements to improve your QIA rates. Feel free to schedule a call/webex/in person site visit with Network staff to discuss how your facility can work to achieve CMS set goals.
