#3. Review RCA/PDSA Is your facility making any progress in overcoming your initial Root Cause for low Transplant or Home Modality rates? You will be asked to list two forms of education that you use for your QIA(s). Are they effective? Do you need new or additional materials?

#4. Patient Involvement in QAPI
Please discuss QIA progress and patient feedback in QAPI. Best Practice is having a patient present their activities/involvement in the QIA(s) at the beginning of the QAPI meeting - either in person or on the phone.
#5. Submit Monthly Status Report Online by 4/30/2019
Our work in April will be to build on the activities of the past months and work with your patients, family members, care partners, facility staff and others to improve transplant and home therapies rates.
At this point (April), you should be formally tracking your patient education efforts, and trending their interest and feedback regarding Home Therapies and/or Transplant. (Depending which QIA your facility is participating in)
Boundrilessness at the Network:We are working with other Network Programs, Donate Life, and other stakeholders to identify resources and practices that can help you and your patients.
National Donate Life Month: You may have already seen the Donate Life Month e-mail. (Pro-tip - any activities for Donate Life (like using the coloring sheet linked above) would be considered a patient engagement activity)
Join the Health Care Quality Improvement Community at JENY 2.0 : Help us answer the question "Why does New York State (NW2) only have 3% (1986/29851) patients using Home Therapies?" Give us your answer to the question - so that together we can work on a solution.
