Diseases of the Esophagus Special Collection on Eosinophilic Esophagitis


The Diseases of the Esophagus (DOTE) Journal is delighted to present this Special Collection on Eosinophilic Esophagitis with Evan Dellon serving as the Guest Editor. 


Over the past few decades, we have seen a rapid rise in understanding of this disease. This special collection showcases knowledge gained from the spectrum of EoE with studies on epidemiology, biomarkers, clinical presentations, treatments, and gaps to be addressed. Effect of benralizumab on histopathology and inflammatory signatures in a clinical cohort of eosinophilic esophagitis


The preliminary findings of this recent phase 3 trial of benralizumab, a monoclonal antibody that binds to interleukin-5 receptor alpha (IL5Rα), in patients with eosinophilic esophagitis (EoE) showed that while benralizumab significantly reduced tissue eosinophilia and improved endoscopic and histologic disease scores, it only provided partial symptom improvement. Despite successful eosinophil eradication, histologic analysis revealed persistent epithelial features of disease including basal cell hyperplasia and dilated intercellular spaces. Gene expression profiling confirmed that benralizumab-treated patients maintained transcriptional signatures similar to active EoE, with upregulation of inflammatory markers and downregulation of epithelial barrier genes. The researchers concluded that interleukin-5 receptor alpha blockade alone is insufficient to control EoE, as it fails to resolve the underlying epithelial dysfunction and inflammation despite eliminating eosinophils.
