It is an important day to honor and show appreciation for ALL who have served in the United States military—in wartime or peacetime, living, or deceased.

211 San Diego has an upgraded Community Calendar where you can share your events for free!
New Features:
- Streamlined Event Submission: Easily submit one-time or recurring events and create event series.
- Enhanced Event Listings: Add eye-catching images and detailed, searchable descriptions.
- Virtual or In-Person Details: Include links or addresses.
- Event Management Dashboard: Keep track of all your events, and edit and update existing listings as needed.
- Share with Ease: Once your event is published, access the unique link to share with your network.
To start submitting events, you must first create an account. Events that align with the inclusion requirements will be published.

Activate Your Holiday Assistance Programs and Resources
The holiday season is here!
During the holiday season, community partners across San Diego offer programs that help make the holidays possible for individuals and families who would otherwise not be able to celebrate.
Families are already searching for services that offer access to holiday meals and festive activities.
If you provide services that can help bring joy to their celebrations, make sure to activate, add, or update your holiday offerings. To connect clients to services that can help, browse through the list of available programs in the link below.

Fight Hunger This Season
There are many ways to get involved this holiday season at the local Albertsons, Vons, and Pavilions.
From now until November 28th, people can donate holiday meals at checkout through the Nourishing Neighbors campaign. Donate much-needed nonperishable food items in our famous red barrels at your local Albertsons, Vons, or Pavilions and participating partner locations throughout San Diego County. $5 Hunger Bags will also be available to donate this holiday season, starting December 1st!

Leveraging Collaboration To Demonstrate How To Effectively Prevent Homelessness
In 2019, 211/CIE San Diego and the Salvation Army secured funding from The Regional Taskforce on Homelessness (RTFH) to establish a collaborative initiative of 11 service providers with the goal of taking advantage of a one-time state funding to do cross-organization care coordination to demonstrate the essential impact homelessness prevention programs have on keeping people housed. In fact, because of the system-wide data that was able to be collected using the legal and technical infrastructure of the Community Information Exchange (CIE), the initiative’s efforts showed that within
the first 30 days, those enrolled in the homelessness prevention programs were over 15 times less likely to become homeless compared to those who were not enrolled.
The initiative’s care coordination goals were to streamline eligibility screening, and program access, and capture reportable outcomes for impact analysis. An essential source of analysis data came from the Regional Task Force on Homelessness (RTFH) through their longstanding partnership and data sharing with the CIE.

Transforming Community Care through Shared Client Records
The San Diego Region Data Infrastructure Asset Map is a 2024 asset map of the region’s data infrastructure and a collaborative roadmap to guide stakeholders’ efforts to effectively and appropriately leverage healthcare and social services data within the ecosystem to support improved care coordination, with a specific focus on Medi-Cal (Medicaid) transformation initiatives. An Advisory Group of cross sector subject matter experts participated in the development of this report that includes an overview of the current and desired future-state of local data sharing, policy implications, use cases, and recommendations.

Payment & Data Exchange Standards: Gravity Project Discovery Findings
Beginning in February 2024, the Gravity Project convened more than 20 cross-sector stakeholders to complete a six-week discovery project regarding social care payments data and exchange standards. The project sought to understand and define the existing landscape for community-based organization (CBO) reimbursement and identify the components of the CBO reimbursement process that would benefit from national standards to be developed by the Gravity Project.

The CIE Partner Network works to improve the overall service delivery system for clients. Our meeting provides a forum for partners to share updates about ongoing CIE Initiatives, share trends in our community, and collaborate on how we as a network are leveraging our partnership, data sharing, and the CIE technology platform to address the social determinants of health and increase health equity.
Join us for a range of content. Each month, we may dive into the latest technology projects, elicit feedback on the platform’s care coordination tools, or spotlight programs and supports for unique populations. The partner gathering is a space to engage with peers across our shared network. Join us on the 4th Thursday of every month (except November and December) at 9:00 am via Zoom.
