The Diabetes Prevention and Diabetes Management program staff in the Community and Clinical Connection for Prevention and Health Branch (CCCPH) at NC DHHS are joining forces to offer a new quarterly learning collaborative. The collaborative will be geared toward pharmacists who are currently offering or are interested in offering diabetes prevention and management programs to their patients and communities. Anyone who works in the pharmacy setting, pharmacy students, or those with interest in pharmacy work are welcome to attend.

To ensure lifestyle change programs are expanding their reach and finding champions to drive program engagement, the Community Health Worker and Community Champion Promotional Guide was developed to assist with recruiting efforts. This resource is built with programs in mind and intended to be customized by CDC-recognized lifestyle change program organizations. These organizations can insert relevant local information in the gray highlighted areas to customize materials for their needs.
