What Are the Challenges in Moving Along Value-Based Care in Rural Communities?

Value-based care has been cited as a way to reduce healthcare costs and improve outcomes. However, adoption has been slower in rural communities, with rural providers facing several uphill challenges, according to one health company executive “Providers I’d say have been a bit more resistant in rural areas to take on risk, to take on value-based care. And it’s understandable. Many of them are struggling to stay afloat, stay viable,” said Amar Kendale, co-founder and president of Homeward Health, during a recent interview at CES 2024. “So they’re really optimizing what they know well, which is fee-for-service, in order to do that. There also aren’t the same kind of market pressures that you have in urban or suburban areas where the health plans can be a bit more assertive in terms of pushing their networks towards value.”

New CMMI model integrates physical and behavioral health

The Centers for Medicare and Medicaid Services has announced a new model to test approaches for addressing behavioral and physical health, as well as social needs, for people with Medicaid and Medicare. The model will launch in the fall 2024 and is anticipated to operate for eight years in up to eight states. CMS is expected to release a Notice of Funding Opportunity this spring. Practice participants in the Integration for Behavioral Health Model will be community-based behavioral health organizations and providers, including Community Mental Health Centers, public or private practices, opioid treatment programs, and safety net providers where individuals can receive outpatient mental health and SUD services.

What Drove Success for ACO REACH’s Highest Performing ACOs in 2022?

On October 23, CMS released the results and public use file for Global and Professional Direct Contracting (now known as ACO REACH)’s 2022 performance year. The program had 99 participating ACOs in 2022, of which 81 ACOs attained gross savings and 75 ACOs attained net savings (after applicable discounts, CMS shared savings, sequestration, and risk corridors). How value-based care can address health inequities

Often the key to solving a troublesome problem is approaching it from a different direction. As 19th-century mathematician Carl Gustav Jacob Jacobi wisely noted (and as popularized by renowned investor Charlie Munger), “Invert, always invert.” Therefore, when we consider ways to transform our current health care system in the U.S. to deliver more reliable, equitable, cost-effective value, we should explore the factors that perpetuate a continued unreliable, wasteful system that propagates inequity. Let me elucidate. The prevalent FFS payment model in the U.S. health system inadvertently fuels health inequities by incentivizing the volume of services over outcomes. It fails to account for and address underlying disparities in health care access and the impactful influence of culture and social determinants of health (SDoH) — both significant obstacles to well-being.

Phoenix-based Equality Health offers value-based care solutions dedicated to serving independent primary care practices in closing gaps in care. Last June, I had the opportunity to interview Mark Stephan, M.D., M.B.A., the company’s chief medical officer, about Equality Health’s ’Medicaid-first” business model. “The way we enter markets is in the Medicaid space, but we also have Medicare Advantage contracts and we’re in ACO REACH,” Stephan told me. “It’s important to bring a portfolio of value-based agreements to a practice. It helps with mindshare and behavior change and practice transformation when you have a significant part of your panel in a value-based agreement.”
