Learn about the upcoming Nurse Assessor Program that is planned to begin in July 2025. We will review the background of this program, outline the process, and provide basic information on what stakeholders can expect moving forward.

Buy-in Program for Working Adults with Disabilities Stakeholder Meeting

The Health First Colorado Buy-In Program for Working Adults with Disabilities allows adults with a qualifying disability "buy into" Health First Colorado, Colorado's Medicaid Program. Members who work and earn too much to qualify for HFC may qualify for the Buy-In Program. Interested stakeholders are invited to one of two virtual stakeholder meetings to learn about updates being made to the Working Adults with Disabilities Buy-In Program effective July 1, 2025.

At these meetings, HCPF staff will present an overview of the changes being made to how Health First Colorado members enroll into the Buy-In Program for Working Adults with Disabilities. These changes will specifically impact Health First Colorado Members who also receive services through Home and Community-Based Services (HCBS) waivers. Attendees are invited to learn and ask questions about the upcoming changes. These two meetings are identical and therefore attendees only need to choose the meeting date and time that best fits their schedule.

✴️ Community First Choice (CFC) Council Meeting

The CFC Council is in place to collaborate with and advise HCPF on the design and implementation of a CFC option in Colorado. Primarily made up of individuals with disabilities, those 65 years or older, and their representatives, the council’s experience and expertise will help ensure that CFC is implemented in a way that best meets the needs of Coloradoans with disabilities.

Transitions Stakeholder Advisory Council (TSAC)

Welcome to the TSAC monthly meeting. This is the place to talk openly about challenges and solutions related to Transition Services and programs. You can ask questions and get to know fellow options counselors, providers, transition counselors and everything in between that support community transitions.

Training and Technical Assistance

New

Office Hours/Work Group Sessions for Case Management Agencies (CMAs)

HCPF is hosting office hours/work group sessions so CMAs can meet with HCPF staff and learn about At-Risk Diversion. During these times, HCPF staff will provide clarification and answer questions regarding At-Risk.

At-Risk Members and their chosen supports will receive a targeted outreach to evaluate and/or re-determine service needs, available resources, and potentially any additional supports, services, or referrals to mitigate risks and assist with keeping Members safe in a community setting of their choice. This new process will include targeted outreach to Health First Colorado Member(s) living in the community who are determined to be At-Risk. HCPF will request a generated list of Members with the highest risk for admission to an institutional setting such as a nursing facility based on their risk score and it will include certain characteristics, diagnoses, and/or behavioral needs.

Case Managers and any other interested stakeholders can join this session.

Starting in February 2025, Office Hours will occur on the third Tuesday of each month from 2 to 3 p.m. These events can be found on the OCL Stakeholder Engagement Calendar.
