Coastal Horizons Adolescent Services

Wilmington Health Access for Teens offers comprehensive primary medical care, mental health and prevention services to children, adolescents and young adults ages 6-24.

Their School-Based Wellness Centers an integral part of the Wilmington Health Access for Teens (WHAT) program. These centers are beacons of hope for students, offering an innovative blend of primary care and behavioral health services within the familiar environment of their high schools.

School-Based Wellness Centers provide expert behavioral health treatment to address a wide range of challenges, including anxiety, depression, adjustment stressors, trauma, and impulsivity. Even during school breaks, their offerings include individual, family, and group therapies, as well as medication evaluation and management by a dedicated team of licensed professionals.

Individuals continue to receive WHAT services and seamlessly transition to the main clinic up to age 24, fostering continuity of care into young adulthood.

One success story perfectly illustrates the impact of our School-Based Wellness Centers. A female student struggling with moderate depression and anxiety embarked on her journey at the Laney Wellness Center, seeking outpatient therapy. After a few sessions, her therapist recommended a consultation with our physician assistant to explore the possibility of prescription medication as part of her treatment plan.

In a compassionate and informative session, the student and her parent/guardian sat down with the physician assistant. They received thorough medication education and had all their questions answered, empowering them to make an informed decision. Together, they decided that a trial of prescription medication would be a valuable addition to her treatment.

The physician assistant's dedication didn't stop there. To ensure the student's well-being and monitor for potential side effects, follow-up appointments were conveniently scheduled within our school-based wellness center. This approach ensured the student's education remained uninterrupted and prevented the parent from needing to take time off work for future visits.

This story not only showcases the positive impact on individual well-being but also highlights how our School-Based Wellness Centers are breaking down barriers to care, offering accessible and family-friendly services that promote well-being and academic success. At Coastal Horizons Center, we're dedicated to transforming lives and fostering continuity of care from adolescence into young adulthood.

Advanced Suicide Risk Assessment:

Intervention and Postvention

This advanced training will equip behavioral health and healthcare professionals with information about suicidality that was not taught in most academic training or supervision, and that most professionals haven’t routinely incorporated into their practice.

The presenter will provide updates on the most current research and practices related to suicidology. Participants will be able to proactively identify risk and assertively act to reduce risk prior to a suicide attempt. Postvention responses after a suicide or a suicide attempted will be discussed during the training.

Professionals who have experienced death by suicide of a patient become at increased risk for suicide themselves; therefore, self-care strategies will be introduced. This training will lay the foundation for assessment protocol and how to be strategic in choosing the most appropriate interventions.

The presenter will introduce a model that will address the anxiety experienced by many professionals when talking about suicide risk with patients and provide guidance to implement common sense interventions.

Trillium receives $1.6 million for High Fidelity Wraparound services

Trillium Health Resources received a $1.6 million allocation from a Substance Abuse and Mental Health Services Administration (SAMHSA) grant that the North Carolina Department of Health and Human Services (NCDHHS) Division of Child and Family Well-Being (DCFW) has been awarded. This allocation will expand High Fidelity Wraparound (HFW) within our region, support a dedicated HFW liaison at Trillium, add two new HFW teams, and provide support for an existing HFW team within our region.

HFW assists youth and their families to achieve the changes they want in their lives. The interactive, team-based approach employs ten principles and four phases to help families reach their desired outcomes and develop the skills needed for the future. The support teams in HFW include a coach, up to four wraparound facilitators, and three support partners. The youth served by HFW have multi-system involvement and complicated challenges. HFW seeks to intervene before these challenges can lead to the use of restrictive levels of care, high costs, and poor outcomes.

“Trillium welcomes this additional funding so that we can support even more people with HFW,” shared Leah Smith, Innovative Development Director at Trillium. “In the past year, more than 35 families have neared completion of HFW services. They have reported increased communication, learned about community resources, and decreased their need for crisis responses due to plans put in place. This funding will increase access in our area to collaborative HFW teams that partner with youth and families to develop and achieve their own unique family visions, with the support that youth and families want and need.”

Substance Abuse and Mental Health Services Administration (SAMHSA) Award

In the lead Up to Prevention Month, the Biden-Harris Administration awarded $42.6 Million to expand and strengthen the capacity of States and Local Community Prevention Providers to Implement Evidence-based Prevention Strategies. SAMHSA offers a Prevention Month toolkit, with downloadable resources, including prevention information for families, evidence-based practices for communities, and technical assistance for grantees.

Halifax County Playground Grand Opening

“I’ve never been able to do stuff that other kids can. It’s like a dream come true to me. I am so glad this happened.” These were the words of 12-year-old Emma Shifflett at the opening of the Greenville inclusive playground. Over 30 playgrounds across Eastern North Carolina have given all kids the chance to play.

Play is essential to development. It contributes to the cognitive, physical, social, and emotional well-being of children and youth. Communities coming together in shared spaces is especially important to reduce the presence of childhood adversity. Playgrounds give the space for mutual joy and shared communication for both children and their parents.

Trillium’s inclusive playgrounds eliminate barriers to play for individuals of all ages and abilities! Inclusive playgrounds are fully accessible, have adaptive equipment such as a Liberty Swing for wheelchair users in some locations, and all have equipment designed with physical and sensory benefits in mind.

Children with disabilities are able to play alongside their peers to develop social and emotional skills—experiencing the joy of play for themselves and not just watching. Parents or caregivers, including veterans with disabilities and grandparents, can also play alongside their child on a universally designed playground. A playground is more than slides, swings, and structures. When everyone can play together, these structures help build and bolster the physical, social, and mental health of our communities.

Our latest inclusive playground opened in Halifax County on October 10, 2023. We encourage you to visit the playground closest to you.

Trillium Re-Entry Simulation

The Training Department hosted its second Re-Entry Simulation event on September 26 in Greenville. The event featured a hands-on experience of barriers individuals often face when released from incarceration. Empathy is a powerful tool to change. The simulation is part of a larger effort to ensure individuals released from incarceration have the services and resources to successfully lead fulfilled lives and lessen recidivism rates. Trillium is proud to work in collaboration with community partners such as law enforcement and providers. Additional events are coming, therefore please be on the lookout for those opportunities to join us.
