Starting to Prepare

Community Pharmacy & the Delivery Plan for Recovering Access to Primary Care – Bulletin 3

This is our third bulletin in this series relating to the announcement in November of the launch of the new Pharmacy First Advanced Service as well as other changes to pharmacy funding and services. In our first bulletin we introduced the new Pharmacy First Advanced Service, (see here), with our second bulletin focusing on the expansion of the Pharmacy Contraception Service (see here). This third bulletin aims to get pharmacists and pharmacy teams to start to think about preparing for the start of the Pharmacy First Advanced Service (expected 31 January 2024).

First Things First – Have You Registered for the Service & Claimed the £2000 Initial Fixed Payment?

We would like to encourage all pharmacies to sign up to deliver the Pharmacy First Advanced Service. This is a great opportunity, is well funded, and will lay the foundations for future clinical services, establishing community pharmacy as an increasingly vital part of primary care.

Remember, the deadline for registering for the service to receive the £2000 payment is 31st January 2024. Pharmacies who register for the service after 31st January 2024 can still sign up for the service but will not receive the £2000 initial fixed payment. See the Community Pharmacy England (CPE) news story here.

Getting Ready to Provide the Service

CPE has published the following helpful resources to support pharmacies and pharmacists to start to prepare for the service:

These checklists detail the actions pharmacy owners and pharmacists (including locums) can start to undertake to prepare to provide the Pharmacy First Service, while waiting for further information and resources to be published. Have a look at the checklists and start to think about what you will need to do to be ready for the start of the service.

Understand the service requirements by reading the service specification, clinical pathways, draft Patient Group Directions & protocol as well as the FAQs on the CPE Pharmacy First webpage here.

Think About……

  • Training

Start to make a training plan with the pharmacists, (including regular locums), who will be providing the service to ensure that when the service starts, they are competent to provide the service.

Review the CPPE self-assessment framework for Pharmacy First to assess knowledge in relation to the service and to support in identifying any learning requirements. Have a look at the CPPE resources available on the CPPE Pharmacy First page to see if any of these would help prepare to provide the service.

Free Clinical Examination Skills Training for Community Pharmacists (CliniSkills) – have a look at https://www.cliniskills.com/community-pharmacists/ to see the training offers available. CliniSkills offers several modules which will help prepare for Pharmacy First.

Otoscope Training - pharmacists providing the service will need to be competent in the use of an otoscope (except for Distance Selling Pharmacies). CPPE is holding several face-to-face ENT Clinical Assessment Skills Workshops, however, places are limited with the first set of local events already fully booked. We are currently working with commissioners and CPPE to plan a limited number of additional face-to-face ENT events and will let contractors know the details as soon as these are finalised. Please make sure you are regularly checking your pharmacy NHSmail for updates and if you have not done so already, sign up to our mailing list here so you are made aware of any local training events on the service.

CliniSkills is delivering an online webinar for otoscope training on Thursday 18th January 2024, 18:00 – 19:30. Pharmacists attending this webinar will first need to complete a gateway module AND ENT module before the webinar to get maximum benefit from the training. Find out more and sign up here.

Virtual Outcomes training is expected to be launched early January 2024 and will include a module per PGD.

  • Equipment

You will need an otoscope to provide the service (this does not apply to distance selling pharmacies). Think about ordering your equipment sooner rather than later as this may be required when attending any training (guidance on selecting a suitable otoscope can be found in Annex C of the service specification).

Round up of Latest News on the Service

Finally, just a reminder that Pharmacy First builds on the existing Community Pharmacist Consultation Service (CPCS) and once it is launched, CPCS will cease to exist (but patients can still access the service when they are appropriately referred under the Pharmacy First Service instead). Pharmacy First will enable community pharmacists to supply more treatments for a wider range of conditions which is great news and will help practices even more with access but it’s important that practices continue to send referrals to Pharmacy First once the service is launched.

It would be great if pharmacy teams could engage with their local practices and keep them updated about the forthcoming changes to CPCS. Please see this helpful one-page summary on the Pharmacy First service which could be shared with local practices; Briefing for LMCs & GP Practices on Pharmacy First.

Good relationships between healthcare professionals will be a key factor to ensuring the service is a success and we encourage pharmacies to actively engage with their local practices both now, before the service commences, and throughout the implementation stages of the service to keep them updated of progress in relation to service commencement.

Please visit the CPE Pharmacy First webpage for further information and details of this new service.
