To better understand the state of the Social Determinants of Health – the conditions in which people are born, grow, live, work, and age – and how they are shaping the health of North Dakotans and the communities in which they live, a Social Determinants of Health toolkit was released in 2023. The toolkit is a supplement to the report “Health and Well-Being in North Dakota, 2022” sponsored by the Blue Cross Blue Shield of ND Caring Foundation and presents county-level indicators. The toolkit’s data and graphics have recently been updated and will continue to be updated regularly as new and current information become available.
