Required Nomination Materials

Collecting Nomination Materials:

To submit a nomination, you will need to collect the following materials

to be entered or uploaded to our online registration form:



Basic Background Information

You will be entering your contact information, basic student info (grade level, school, etc.), and parent/legal guardian contact information

in the online nomination form.

Signed Permission Form from Parent or Legal Guardian

Print out the following parental permission slip to be filled out and signed and collected for all students who have not turned 18 by this year's

submission deadline.

Three to Five Recommendation Letters

From teachers, administrators, guidance counselors, organizational or civic leaders, or others familiar with the student's abilities and achievements.

(The person submitting the nomination can write one of the letters.)


Letters should include:


  • The length of time and capacity in which you know the student.


  • Descriptions of the student's achievements in which they have exceptional abilities.


  • Unique examples placing the student clearly above other high-ability students.


  • The student's positive impact on others.



  • Appraisal of a potential positive future for this student.

Student Essay

Students should include:


  • How they became interested in the area(s) for which they wish to be recognized.


  • Why they think these areas are important.


  • The impact their activity/efforts has had on themselves and others.


  • Their future plans and goals.


( 750 words maximum, to be uploaded in a pdf format.)

Up to Five Additional Supporting Documents

(that may help the judges in their decision.)


Examples could include news releases, or copies of student's original work or research, or other items related to the award area(s) for which they wish to be considered. Items should be in pdf or as a png image, as appropriate.


Items that are web-based, such as online publications or links to video projects can be listed in a document with live links to the materials.


As You Begin Collecting Nomination Materials....


Please note that all of the required items will be uploaded though a Google Form

that requires you to be logged into a Google-based account to upload materials.


If you do not currently have access to a gmail or Google-based Organizational Account, you will need to set one up in order to upload the required information into the nomination form.


To expedite the evaluation procedure, each individual item should be uploaded separately into specific areas of the nomination form.
