In May, we reflect and celebrate the important role that Asian Americans, Native Hawaiians, and Pacific Islanders (AANHPIs) have played in our shared history.
The Federal Asian Pacific American Council announced the theme this year, Advancing Leaders Through Innovation which celebrates the culmination of the 2021-2024 series highlighting our nation's efforts in “Advancing Leaders”.

Military Appreciation Month | |
May is Military Appreciation Month, a special time for those in and out of the military.
This month we honor and recognize the contributions, sacrifices, and service of the members of the armed forces, past and present.
Not only do we pause on Memorial Day to remember the sacrifice and service of those who gave all, but the month also marks several other military anniversaries and events, including Military Spouse Appreciation Day and Armed Forces Day.
Here are some ways you can observe and honor those who have served and their families:

May is CalFresh Awareness Month in California and the San Diego County Health and Human Services Agency along with its community partners will be at several community events where people can receive free screenings and application assistance for CalFresh.
CalFresh is California’s version of the federal food assistance program, SNAP, and has been around since 1964. The program is designed to help families and individuals increase access to fresh and healthy food, part of the County’s Live Well San Diego vision for healthy, safe, and thriving communities.

Z Codes
Z-codes are the bridge between social health data and physical/mental health data and now partners can associate these standardized healthcare codes to Direct or Informational referrals.
By using Z-codes, partners can flag members as potentially eligible for programs and services. This can also support getting authorization to receive Enhanced Care Management and Community Supports services.

Appropriateness of
Referral Notes
This year, we're focusing on improved referral quality and the need to understand why a Direct Referral didn't result in a connection to services. Based on common questions and feedback from partners, we updated the Referral Management process. Referral Notes are now required if someone marks a Direct Referral as not appropriate. This will help teams better identify referral quality trends and process improvement opportunities.

Updated Referral
Detail Layout
The detail view of Referral records has an updated layout including information about the assigned case manager and clearer labels for notes fields. To ensure data quality, this layout is now Read-only.

CalAIM Screening
This new screening tool lists out the various eligibility criteria across all CalAIM programs, reviews qualifying criteria for each program type and helps the user navigate to the right type of program the client is eligible for. The screening is still being piloted but is open for feedback and training is forthcoming.

Recuperative Care Screening
This new screening tool was developed in partnership with the Hospital Association, Medi-Cal managed care plans, and San Diego County's local non-profit recuperative care providers - Father Joe's Villages, Interfaith Community Services, and PATH. The screening is currently in its piloting phase and can be completed only by specific CIE users.

Housing & Food Assessment
The pathway to the Housing & Food Assessment previously found in "Domains" has been moved to the Questionnaire's tab to simplify how CIE Partners can access it through the Partner Community.

Interfaith Communtiy Services
Project Care
Project CARE (Community Action Reaching the Elderly) identifies and supports frail older adults and/or individuals with physical or mental disabilities to prevent social isolation, loneliness, neglect, and abuse by providing a wide range of services. Services include daily reassurance phone calls, referrals to appropriate community resources such as Minor Home Repair, and the Sheriff’s Take Me Home Program, home visits to assess needs, follow-up support services, and reassurance calls made via an automated phone system or staff.

Say San Diego
Provides case management, visit coaching, clinical assessments, in-home parenting, and parenting classes to adults/families where there is risk of child abuse. Services can be provided on-site or in the home. Service priority is given to individuals with active CWS cases that are referred to the program by the County.
Provides a 16-week, group-based education/parent support to foster parents/relative caregivers who are experiencing behavior problems with a child in the home.
Offers help with challenging child behaviors, receives support from other Foster Parents and Caregivers, and earns up to 8 hours of annual training requirements. Free on-site childcare and weekly incentives for participants.
Assists dads by providing new insights and parenting techniques that enable them to gain new skills and tools to support their family unit regardless of circumstance. Discipline, communication, healthy lifestyles, financial security, job readiness, and other themes are covered throughout the program.
All fathers, including civilian fathers, active duty fathers, veteran fathers, and fathers-to-be, are welcome to join for free.

Basic Computer Skills Education and Training, San Diego Futures Foundation
Offers a variety of digital literacy topic training, including adaptive technology for the visually impaired, as well as basic computer training via Zoom. Topics include: • Internet safety • Web security safety • Social media security • Microsoft Office and Google Suite • Smartphone basics • Digital resource assistance

Digital Literacy and Basic Computer Skills Training
Offers free basic to intermediate digital literacy computer classes to San Diego County residents. The basic computer skills training sessions vary in levels. Training sessions are provided at designated City libraries and rec centers. Occasional off-site and online sessions are available based on participants' needs.

Emerging Models of Community-Led Care Infrastructure
Join us for the 8th Annual CIE Summit! This hybrid event will be hosted in person at the University of San Diego campus’ Hahn University Center. This year will kick off with a pre-conference CIE 101 session on June 24th and two full days of engaging sessions on June 25-26.

This year’s content will be focused on sharing innovations in designing and implementing community-led care infrastructure from emerging CIE models across the nation. Workshops will focus on lessons learned, best practices, and practical applications across early to late stages of planning and implementation. Cross-cutting topics will focus on:
- Centering Lived Experience
- Healthcare In Action
- Aligning with Policy

The CIE Partner Network works to improve the overall service delivery system for clients. Our meeting provides a forum for partners to share updates about ongoing CIE Initiatives, share trends in our community, and collaborate on how we as a network are leveraging our partnership, data sharing, and the CIE technology platform to address the social determinants of health and increase health equity.
Join us for a range of content. Each month, we may dive into the latest technology projects, elicit feedback on the platform’s care coordination tools, or spotlight programs and supports for unique populations. The partner gathering is a space to engage with peers across our shared network. Join us on the 4th Thursday of every month (except November and December) at 9:00 am via Zoom.

Learn about U.S. Citizenship and prepare for the citizenship test!
San Diego County Library partners with local non-profit Jewish Family Service of San Diego to offer free citizenship classes at select library branches. Our partners provide free application help from accredited immigration specialists.
Summer 2024 Classes: June 18-August 22, 2024
Free 10-week citizenship classes will be held at the following library branches:
- El Cajon: Tuesdays from 10 a.m.-12 p.m.
- Spring Valley: Wednesdays from 4 p.m.-6 p.m.
- San Marcos: Thursdays from 10 a.m.-12 p.m
- Imperial Beach: Thursdays from 4-6 p.m.
