Value-based healthcare, the holy grail of American medicine, has three parts: excellent clinical quality, convenient access and affordability for all. And as with the holy grail of medieval legend, the quest for value-based care has been filled with failure. In the 20th century, U.S. medical groups and hospital systems could—at best—achieve two elements of value-based care, but always at the sacrifice of the third. Until recently, American medicine lacked the clinical knowhow, technology and operational excellence to accomplish all three, simultaneously. We now have the tools. The only thing missing is “system-ness.”
The advent of value-based care, based on the Institute for Healthcare Improvement “Triple AIM” framework (improving the health of populations, reducing the per capita cost of care and improving the individual experience of care) has motivated providers to shift their focus from volume to value. And, as providers shift to value-based activities, their reimbursement model is also more in alignment with payers. Like payers, providers now benefit from a fundamental understanding of and ability to manage risk.
Quality and quantity have a nuanced relationship in the healthcare system: put simply, payers want to reduce members’ quantity of low-value services while increasing the number of services that produce better quality care. As a result, utilization measures that track the use of high- and low-value care are critical to a value-based structure. The triple aims of value-based care—improving member experience, population health, and cost per capita in healthcare—all draw on utilization measurement data.
Integrating episodes of care within your population health infrastructure can drive higher value specialty care and increase savings earned. While CMS aims to support more coordination between bundles and ACOs, organizations should set a specialty care strategy that works for them. Specialty care accounts for a disproportionate share of healthcare expenditures in the US, driven in part by remarkable advancements in diagnosing, preventing, and treating serious illness. Responsible for an estimated $2 trillion dollars annually and 63% of all medical expenditures, specialty care is also a substantial source of variation in quality, outcomes, and access.
