CELEBRATIONS AND OBSERVANCES

Black History Month


In February, we honor and celebrate the achievements and contributions of African Americans who have shaped our history and culture.


According to The Association for the Study of African American Life and History, the theme this year is "African Americans, and Labor" which focuses on the various and profound ways that work and working of all kinds – free and unfree, skilled, and unskilled, vocational and voluntary – intersect with the collective experiences of Black people.

DIRECT REFERRAL SPOTLIGHT

2-1-1 VITA & CalEITC Navigation


211 San Diego can educate clients about potential tax credits, including CalEITC, EITC, and YCT, while screening callers for eligibility for free tax preparation appointments through VITA tax sites.


To send a direct referral to this service:

  1. Login to Partner Community: https://211.my.site.com/s/login/
  2. Review client record for to ensure the client meets eligibility criteria.
  3. Launch Find Referral from the client record and enter “2-1-1 VITA & CalEITC Navigation” in search bar.
  4. Select 2-1-1 VITA & CalEITC Navigation.
  5. Submit Direct Referral and include details of client situation in the note of referral.

CalAIM Navigation


2-1-1/CIE San Diego offers CalAIM Navigation Services for Adults and Children with Medi-cal who may be eligible to receive access to new and improved services to get well-rounded care that goes beyond the doctor's office or hospital and addresses all of their physical and mental health needs. 



These changes are part of a broad transformation of Medi-Cal to create a more coordinated, person-centered, and equitable health system that works for all Californians.

CIE Utilization Reports


The CIE Utilization Reports have been enhanced to offer a comprehensive overview of your team's performance across 14 distinct activity types and 3 overarching activity groups. This new feature provides not only an aggregated agency utilization score but also breaks down the performance into specific segments.


Moreover, the report offers valuable insights into key utilization indicators that reflect your team's effectiveness in implementing CIE. These indicators include metrics related to adoption like logins to the platform, levels of engagement including page views, and the frequency and success of interventions like sending an managing referrals, giving you a clearer understanding of how well your team is integrating CIE into their daily workflows.

“We Need to Know These Things”: Use Cases for Combined Social and Clinical Data Among Primary Care-Based Clinical and Social Care Providers



Primary care organizations are increasingly collecting data on patients’ social risks, bringing forth an unprecedented opportunity to present combined health and social data that clinical and social care providers could leverage to improve patient care and outcomes. Little is known, however, about how these data could be used and what combinations of specific data elements are most helpful. We explored how primary care staff who provide clinical or social care services view potential benefits of and use cases for combined patient-level clinical and social data.

CIE Partner Gatherings


The CIE Partner Network works to improve the overall service delivery system for clients. Our meeting provides a forum for partners to share updates about ongoing CIE Initiatives, share trends in our community, and collaborate on how we as a network are leveraging our partnership, data sharing, and the CIE technology platform to address the social determinants of health and increase health equity.


Join us for a range of content. Each month, we may dive into the latest technology projects, elicit feedback on the platform’s care coordination tools, or spotlight programs and supports for unique populations. The partner gathering is a space to engage with peers across our shared network. Join us on the 4th Thursday of every month (except November and December) at 9:00 am via Zoom.


The next CIE Partner gathering is on February 27, 2025
