CSRO Raises Concerns, Recommends Delay of RA Cost Measure


Given concerns about a newly developed cost measure for rheumatoid arthritis (RA), CSRO requested that it not be recommended for use in the Merit-based Incentive Payment System (MIPS) program at this time. In CSRO’s most recent communication about the flawed measure to the Partnership for Quality Measurement (PQM), CSRO said: “[u]ntil rheumatologists have unfettered access to all available treatment options for RA without interference from third party cost control policies, it is inappropriate to hold rheumatologists accountable for the costs of the medications that beneficiaries require.”

CSRO Seeks Revisions to Medicare Advantage Biosimilar Substitution Proposal, Plan Marketing


In comments to the Centers for Medicare and Medicaid Services (CMS) on its recent proposals to modify Medicare Advantage and Part D plan requirements in 2025, CSRO registered concerns and opposition about a policy that would allow plans to substitute biosimilars and interchangeable biosimilars as “maintenance changes” mid-year. We noted the potential negative impacts on continuity of care and urged CMS not to finalize the policy. 


CSRO supported additional marketing reforms, including those that would address compensation to brokers/agents and ensure adequate notice to enrollees on their use of supplemental benefits. We also implored the Agency to impose stiff penalties on plans that fail to adhere to new marketing requirements, including participation bans for the most abusive plan actors.

Rheumatologists, Providers & Patients Seek Improvements in Exchange Plan Policy Proposals


Last year, the Department of Health and Human Services (HHS) and Centers for Medicare and Medicaid Services (CMS) issued its 2025 Notice of Benefit and Payment Parameters (NBPP), which proposed changes to plans established by the Affordable Care Act (ACA), commonly referred to as Exchange or Marketplace plans. Some of the proposed changes addressed network adequacy, the provision of essential health benefits (EHBs), prescription drug benefits, and non-standardized plan options, among other things. 


CSRO, independently and as part of the Alliance of Specialty Medicine (ASM) and the All Co-pays Count Coalition (ACCC), urged the Agency to better protect access to specialty drugs for consumers in Exchange plans, highlighting emerging challenges with “alternative funding” practices that may diminish access. All three groups also urge the Agency to provide enhanced oversight when it comes to non-standardized plan options in the health insurance marketplace based on concerns that such options might further hinder access to care for those with chronic and high-cost conditions, and to ensure specialty and subspecialty providers are able to join and remain in Exchange plan networks.
