TO ENROLL IN YOUR 2024 BENEFITS,

please follow the instructions below

• Review the 2024 Employee Benefits Guide, which includes the employee premiums for all benefit plans

COMPLETE AND SUBMIT AN OPEN ENROLLMENT ELECTION FORM!  

• To enroll or decline participation in the 2024 flexible benefit plans (required) and to REMAIN ENROLLED in your current benefits, please COMPLETE the form linked below: “2024 Open Enrollment-Flexible Benefits Election/NO CHANGE TO CURRENT BENEFITS” form.

• To enroll or decline participation in the 2024 flexible benefit plans (required) and to CHANGE YOUR CURRENT ENROLLMENT, please COMPLETE the form below: “2024 Open Enrollment-Flexible Benefits Election/Change to Current Benefits” form.  Please note, upon submission of the change form, PLEASE REVIEW IT WITH HUMAN RESOURCES TO ENSURE THE FORM HAS BEEN FILLED OUT CORRECTLY.
