Leadership Transition at Sonoma Connect | Sonoma Unidos

Midway through January we announced that Adriana Arrizon accepted a position as the first Executive Director of Health Action Together and that Saskia Garcia would become our Interim Director. Please see the following messages from Adriana and Saskia and our press release for more information.

Dear Sonoma Connect | Sonoma Unidos Partners,


We know we must continue to hold true to our shared values and accountability to community and that we will make it through this and future changes and transitions, together. I am incredibly grateful to Adriana for her mentorship, creativity, patience, and perseverance. Adriana makes an impact through “walking the walk”, believing change is possible, creating a culture of belonging, and staying relational. Continuing to prioritize equity and accountability to community, as Adriana has done, is foundational to how we must move forward. I hope you will join us in celebrating and continuing to support Adriana’s leadership as she becomes the first Executive Director of Health Action Together!

Black-Led and Equity-driven Local Resources

The Sonoma County Black Forum’s Black Therapy Fund Voucher Program offers up to 12 sessions of mental health counseling, from culturally competent therapists, for up to 234 qualifying Sonoma County Residents. Each session is valued at $150 per session.

Sonoma County Black Forum Financial Assistance: The Sonoma County Black Forum Financial Assistance Program will offer financial support to address essential basic needs of many vulnerable, furloughed, unemployed, and underemployed Sonoma County residents.

Upcoming Action Team Meetings

This month, the Sonoma Connect | Sonoma Unidos Action Teams will be working on the agenda items below:


ACEs Screening Implementation Action Team 

Thursday, February 2, 2023 from 2:00pm-3:00pm

Join us as we look to address ways to connect and incorporate prevention-focused screenings and healing-centered practices that can disrupt and prevent the effects of toxic stress into our community. Please join to help refresh the charter for this year. 


Community Accountability Action Team 

This team will resume regularly scheduled meetings on March 8, 2023, after our four-part Diversity, Equity, and Belonging workshop facilitated by Equity First Consulting concludes. We look forward to resuming meetings and continuing our equity-focused work then. 



Resource Connection Network Action Team  

Monday, February 27, 2023, from 1:00pm-3:00pm

We are currently in the process of training participants of our pilot Resource Connection Network. Join us as we summarize our progress and discuss the next steps of implementation, which include further developing governance infrastructure and understanding the context of this platform within the current funding and data exchange landscape. 


Our Action Team meetings offer simultaneous interpretation in Spanish.

Surveys

We want to highlight a few surveys this month and encourage you to please take the time to fill them. These surveys help determine the allocation of resources and the development of programs to meet the needs of the community. Therefore, we want to stress the importance of taking a few minutes to provide your feedback since it really makes an impact!

Community Action Partnership Sonoma – Community Needs Assessment: 

Community Action Patnership Sonoma invites you to participate in their “Community Needs Assessment Survey” as a way to help them identify where the needs in our community are and create a roadmap to address those needs. The survey is completely anonymous, and does not collect any identifying information. Participation will help their agency and your community strategize for future planning and investment, and identify community needs and goals.

CHW/Promotora de Salud Workforce Survey:

The County of Sonoma’s Department of Health Services’ Community Health Workers for COVID Response and Resilient Communities is administering a Community Health Worker workforce landscape survey in partnership with Sonoma Connect | Sonoma Unidos. Data collected in this survey will be used to inform decision making around Community Health Worker programs and sustainability.

Free Trainings in Sonoma County by Hanna Institute:

In partnership with Sonoma Connect | Sonoma Unidos, Hanna Institute will be offering free training and professional development opportunities in trauma responsive care. They previously sent a survey to assess what type of trainings the community is seeking and are now launching this survey in Spanish. Please help us reach the Spanish-speaking community by sharing the survey below.

Community Partner Events 

Population Health Innovation Lab: Cross Sector Care Coordination Training 

Wednesday, February 15, 2023, 1:00pm 

At this training, Stefani Hartsfield, Hartsfield Health Systems Consulting, will discuss: how coordinated, cross-sector care management reduces barriers to healthcare access and can address social determinants of health; understanding what the facilitators and barriers are to high-functioning, cross-sector care; identifying strategies and tools to address barriers to efficient team-based care; and strategies to improve functionality at a regional level.

Hanna Institute: De-escalation and Crisis Prevention

Thursday, February 23, 2023, 9:30am-12:30pm



This virtual workshop describes everyday communication skills that can be utilized with anyone, anywhere to nurture peaceful crisis resolution as well as detailing more concentrated efforts during an interaction with an agitated individual to de-escalate and support safety. Based on best practices established by trauma-informed and equity-centered practices and psychological science, participants will learn how to assess situations, implement safety measures, apply de-escalation techniques, repair relationships, and debrief after incidents.

SCOE: Youth Mental Health First Aid

Thursday, March 9, 2023, 8:30AM - 4:00PM

*Youth Mental Health First Aid is designed for adults (parents, family members, caregivers, teachers, school staff, peers, neighbors, health and human services workers, and other community members) who regularly interact with young people.*


Youth Mental Health First Aid (YMHFA) teaches caring citizens how to help an adolescent (age 12-18) experiencing a mental health or addictions challenge or is in crisis.

Suicide Prevention: QPR Community Gatekeeper Training

*Open to any Sonoma County community members, including parents, educators, and anyone interested in learning more about suicide prevention.*


QPR (Question, Persuade, and Refer) Gatekeeper Training for Suicide Prevention is a 90 minute educational program designed to teach lay and professional "gatekeepers" the warning signs of a suicide crisis and how to respond.

Structural Racism Explained

This video illustrates how structural racism encompasses systemic, institutional, and interpersonal racism and how they all work together to create racial disparities across entire systems. We want to highlight this video because it is relevant to Sonoma Connect | Sonoma Unidos’ values, which aims to dismantle systems, policies, and processes that generate barriers or oppression and work to generate access to opportunity, wellbeing and self-efficacy for everyone.
