We are excited to be a Premier Partner with Civitas Networks for Health for the 2025 Civitas Annual Conference alongside other California Civitas members. Convening under the theme, “Bridging Data and Doing,” the event will take place from September 28-30, 2025, in Anaheim, CA.

 

The partnership marks a significant collaboration aimed at fostering cross-sector solutions that drive health improvement and innovation across the nation. By leveraging the powerful relationships and collaborative work of 211 San Diego, the organizations will collectively exemplify how exchanging community-level data at the community can drive meaningful change locally, regionally, state-wide, and nationally. There is tremendous opportunity for CIEs, Health Information Exchanges (HIEs), Community Based Organizations, and other health data collaboratives to increase partnerships to further the important connections that can advance whole person care.


Mark your calendars and stay tuned for more information on registration and the full conference agenda. This is a must-attend event for professionals working at the intersection of health information exchange, data-driven health care, and social service delivery.

CIE ON THE MOVE

This past month, the 211/CIE San Diego team participated in various meetings, events, conferences, and more! Here's a summary of our involvement during the months of November and December:


  • November 13, 2024: CIE attended the Live Well SD North Central Community Leadership Team Meeting and learned of the 2024 goals accomplished by the various groups representing different topic/service areas.
  • November 18, 2024: CIE attended the Transportation Justice (ITCH) Meeting hosted by Mid-City CAN to learn of SANDANG & Metropolitan Transit System (MTS) transportation related updates and planning for Transit Equity Day 2025.
  • November 21, 2024: CIE Partner Engagement Director, Zenia Torres & Lived Experiences Inc. Founder & CEO, Oscarin Ortega presented at the Live Well Advance Conference & School Summit on the importance of centering community voice and how it is critical in care coordination.
  • December 2, 2024: CIE attended the Resource & Reentry Hub Industry Day, hosted by the Public Safety Executive Office at the Southeastern Live Well Center. We participated in some activities around what types of supports, providers and people who are justice-impacted, want to see within in a reentry hub, including the hub being centrally located for ease of access.
  • December 3-4, 2024: Members of the 211 SD & CIE Team attended and presented at the Regional Task For Homelessness (RTFH) Conference on Homelessness in Downtown, San Diego, CA. 211SD Chief of Business Development Officer, Camey Christenson, RTFH Regional Director, Hanan Scrapper, San Diego Rescue Mission Chief of Staff, Paul Armstrong, shared practical examples of data sharing to promote care coordination, in addressing homelessness.
  •  December 4-5, 2024: CIE Interoperability & Collective Impact Director, Alana Kalinowski, presented at the 2024 Assistant Secretary for Technology Policy (ASTP) Annual Meeting in Washington, DC.
  • December 7, 2024: Members of the CIE Team were invited to attend Olivewood Garden’s Supporter Appreciation Event for a special celebration in honor of community partnerships. We enjoyed delicious food with ingredients grown in-house and learned more of Olivewood’s impact in the surrounding communities of National City, CA.
  • December 11, 2024: CIE Community Partnerships Manager, Aidee Roman and CIE Partner Engagement Director, Zenia Torres hosted a training session at San Diego Rescue Mission with the SDRM Advocate Team. We also had a glimpse into some of the wonderful programming available to our neighbors and their families this holiday season.
  • December 13, 2024: CIE attended Creative Mornings (SD Chapter) Talks w/Dinah Poellnitz at the SD Central Library. Dinah shared about her work in prioritizing the creation of safe spaces for BIPOC Communities at The Hill Street Country Club in Oceanside, CA.
  • December 17, 2024: CIE Partner Engagement Director, Zenia Torres, had the wonderful opportunity to visit the weekly dinner party hosted by We See You San Diego. These weekly dinners are hosted for guests who may be currently unsheltered/unhoused, to have a seat at the table & enjoy each other’s company, listen to live music, have a delicious meal prepared by loving volunteers (of all ages), an opportunity to personal shop for necessary items, and be celebrated by the larger community! We See You San Diego is intentional in how they build trusted relationships with guests and extending an invitation to change the trajectory of their lives. CIE Partner Gatherings


The CIE Partner Network works to improve the overall service delivery system for clients. Our meeting provides a forum for partners to share updates about ongoing CIE Initiatives, share trends in our community, and collaborate on how we as a network are leveraging our partnership, data sharing, and the CIE technology platform to address the social determinants of health and increase health equity.


Join us for a range of content. Each month, we may dive into the latest technology projects, elicit feedback on the platform’s care coordination tools, or spotlight programs and supports for unique populations. The partner gathering is a space to engage with peers across our shared network. Join us on the 4th Thursday of every month (except November and December) at 9:00 am via Zoom.


The next CIE Partner gathering is on January 23, 2025
