Welcome to the April 2022 SCMR Newsletter. Aligned with the theme for the SCMR 2023 Scientific Sessions of "Changing Global Clinical Practice", this month’s newsletter includes several examples of SCMR’s global mission. The SCMR Translation committee reports on its work and asks for your support, we have news from the first CMR service established in Ecuador, and a webinar series in China. We are a global community, and SCMR is committed to supporting the development of CMR around the world.

SCMR Translation Committee Update
The mission of the Translation Committee is to make key SCMR publications accessible to practitioners in the non-English speaking world. The Committee plays a key role in SCMR’s bigger initiative of promoting global inclusiveness & engagement. The success of the Committee over the years is a result of the tireless work, dedication, and expertise of international SCMR members who translated and reviewed publications in their original languages. Notably, the former chairs, Dr. Lilia Mercedes Sierra-Galán and Dr. Gabriela Liberato, led the committee to expand the world language offerings from 6 to 10 in the last few years.  

In this newsletter, we want to highlight the recent work on translating the SCMR 2020 guidelines on protocols, postprocessing, and indications into Spanish (Katia Menacho Medina, Sara Elizabeth Ramirez-Flores, Diana Isabel Katekaru Tokeshi, and Aylen Perez Barreda), Chinese (Lei Zhao, Meng Jiang, Siqi Tang, Yuanwei Xu, and Yucheng Chen), Turkish (Zehra İrem Sezer and Ahmet Demirkıran), Italian (Anna Baritussio, Santo Dellegrottaglie, Alessandra Scatteia, and Andrea Cardona), Japanese (Masafumi Takafuji, Yasutaka Ichikawa, Takanori Kokawa, and Masaki Ishida), French (Aurélien Bustin and Théo Pezel), and Korean (Heerin Lee and Jung Im Jung). Our additional language offerings will include Portuguese, Romanian, and Persian. All the translated publications are freely available and can be found on the SCMR website.

One additional initiative this year is to include a survey of world language fluency of all members to expand our pool of translators and reviewers. We hope you will respond to the survey and help us extend our reach.

Help SCMR Translate Critical CMR Publications
Thank you to the members of the SCMR Translation Committee for all their efforts translating selected critical publications to extend their global reach in the field of CMR. Visitors can view translated articles by publication title or click on one of the following languages to view papers translated into a specific language: Italian, Japanese, Korean, Persian, Portuguese, Romanian, Simplified Chinese, Spanish, and Turkish.

In addition to these translated publication papers, the Translations Committee has also made contributions to the translation of several presentations from the SCMR "How I Do..." educational series. This series breaks down step-by-step how CMR experts perform a specific CMR protocol. Visit the "How I Do..." series webpage to learn more.

SCMR is looking for volunteers to translate publications. In order to engage CMR professionals around the world, the Society is working to ensure that the necessary education is available in languages other than English. If you are a fluent or near-fluent member capable of providing clinical-level translations, please submit the interest form. There is no deadline for applying. Applications will be reviewed year-round.

CMR in Ecuador
A year ago, for the first time in the Austro region of our country, Ecuador, we started a CMR imaging program to guide clinical decision-making in our population. As a cardiac imaging specialist, I am optimistic about incorporating advanced cardiac imaging techniques into routine cardiology practice.

It is now one year since we performed our first CMR in Cuenca, Ecuador (and we are now up to around 100!) in the "Hospital Universitario del Río," one of the largest hospitals in Ecuador as part of the cardiology and cardiovascular surgery services. My fellowship training in institutions in Argentina and Mexico prepared me to offer this service to our community.

Our initial role is to share experiences and discussions of clinical cases with cardiologists in the region and disseminate the main applications of the technique based on clinical guidelines. Our primary objective is to incorporate CMR imaging into our practice to pursue the highest technical standards in diagnosing and managing cardiovascular conditions. Our goal is to participate as a multidisciplinary team to improve clinical outcomes and the overall clinical experience of our patients.

2022 Scientific Sessions - Claim Your CME & MOC until May 4
To claim CME, MOC, and/or ASRT credit, you are required to complete the evaluation and/or quizzes for the sessions you want to claim credit for. Once you have completed the evaluation, you will be able to download and print your certificate(s).

How to access your learning evaluations and download your conference certificate(s)
You have until 4 May 2022 to view the on-demand content and complete your session learning evaluations to receive your credit from SCMR's 25th Annual Scientific Sessions. Follow the steps below to access your learning evaluations and download your Scientific Session CME Certificates.

  1. Log In to the conference platform.
  2. Select the credit type(s) to claim.
  3. Verify your sessions and complete all necessary session evaluations. They can be found in the task list after logging in.
  4. After you complete your session evaluations, you will be asked to complete an overall evaluation.
  5. Once completed, select "Download/Print Certificate" and your certificate will be downloaded in PDF form.
  6. Do not finalize your CME credits until you have attended/watched all sessions you plan to, as you will not be able to go back into the program.

Be sure to complete all MOC, CME, ASRT, and SAM evaluations necessary before claiming your final total, and make sure to complete your evaluations by 4 May 2022.

The Certification Board of Cardiovascular Magnetic Resonance (CBCMR) examination assesses candidates on their knowledge, skills, and abilities in preparing patients for CMR studies, selecting appropriate protocols, conducting CMR studies in a spectrum of scenarios and interpreting the results, and performing post-processing tasks.

Come On, Everybody, Let's Do the TWIST!

The TWIST (Time-resolved angiography With Interleaved Stochastic Trajectories) angiogram sequence is used to acquire multiphase vascular images. This application requires only a single dose of gadolinium to achieve high temporal and spatial resolution images. The sequence is planned in the coronal orientation with care taken to include all the pulmonary and aortic vessels, and oversampling applied to avoid phase wrap (Figure 1). The patient is instructed to breath gently throughout the acquisition. The contrast injection, which is administered at 2mls per second with a power injector, is started after the first measurement. The number of measurements can be selected by the user. In our CMR lab we use 8 measurements, with each measurement filling the centre of K-space at 8.2sec intervals. Figure 2 shows six images at different stages of the vascular cycle.

The SCMR Introductory Course in CMR (2021 Update)
Participate in the 2021 revision of the SCMR Introductory course in CMR. This course is designed to cover theory, knowledge, and some of the skills required for core training in CMR. The course takes approximately 10 hours to complete and is split into 15 topics. There is a strong interactive component, with questions at the end of each topic, as well as lots of case reviews along the way.

The course is not designed to replicate the practical aspects of scan acquisition and live reporting that can only be gained from working in a CMR department. The course will, however, provide you with 10 hours of theoretical and case-based learning and contribute 30 online cases toward your level I, II, or III certificate.

If you are enrolled in the course via SCMR, a certificate will be provided at the end. Nonmembers will need to sign up as a guest to complete. Registration is free to SCMR members and $100 for nonmembers. To learn more about becoming an SCMR member, visit the SCMR membership page.

22-06: Child with Fulminant Myopericarditis and Tamponade After COVID-19
An 8-year-old healthy male with a past medical history of mild persistent asthma, unvaccinated against coronavirus disease 2019 (COVID-19), was admitted with abdominal pain, emesis, and bilateral hip and thigh pain. He was diagnosed with acute COVID-19 by initial rapid molecular test which was subsequently confirmed by PCR testing with negative antibody testing. He had no preceding fevers. Exam was notable for poor peripheral perfusion. Initial inflammatory markers were normal, and an ECG demonstrated low voltages (Image 1). He required fluid resuscitation both in the emergency department and pediatric intensive care unit but eventually required inotropic support with epinephrine and milrinone. Chest computed tomography showed extensive bilateral lung opacities and atelectasis, consistent with COVID pneumonia (Image 2). Cardiac workup showed elevated troponin (ref <0.029 ng/mL) and BNP (ref <60 pg/mL), with a peak of 2.47 ng/mL and 2,379 pg/mL, respectively. Initial transthoracic echocardiogram showed severe left ventricular systolic dysfunction (LVEF 31%) without left ventricular dilation (LVEDD 3.57 cm, z-score -1.8) with a moderate-sized pericardial effusion (Movie 1). The effusion was globally distributed with a maximum dimension of 1 cm from subcostal imaging.

Submitted cases will be peer-reviewed by the web committee before acceptance and can be listed as a peer reviewed web publication on CVs. Cases published or under consideration for publication elsewhere or that have been previously presented at SCMR Scientific Sessions will not be accepted.
