Hospital costs make up the largest component of spend for any employer-sponsored health plan. An article by The Commonwealth Fund describes four successful collective purchaser-led efforts that significantly lowered prices. The article provides several links to charts and graphics that describe the deteriorating employer landscape.

 

What’s fueling premium hikes in the private health insurance market?


  • Largely rising prices, not greater use of services


  • Consolidation of hospitals and physician practices contributes to higher prices


  • Commercial insurers have also consolidated, but they haven’t used their market power to rein in prices

 

It will require local executives willing to take bold action and sufficient competition to leverage in negotiations. The experiences of those who have achieved success suggest the following lessons for others.

Hospital Price Transparency: A Playbook for Employers


The National Alliance of Healthcare Purchaser Coalitions released this month an employer playbook to help purchasers navigate the data, understand rights and responsibilities as a plan sponsor fiduciary, determine what a fair price is for hospital services in specific marketplaces, and offer guidance about options available individually and collectively to achieve fair pricing for hospital services.

Tips from a Fortune CEO



Mike Critelli is the former CEO and Board Chair of Pitney Bowes, having served from 1997-2008. During his tenure, Pitney Bowes had a reputation as a “best in class” employer when it came to the management of healthcare. Mike is a friend of HBCH and was a keynote speaker at one of the first annual conferences. 

 

Chris Skisak recently asked him his thoughts on what he sees today in healthcare and his advice to other CEOs. Key points in his succinct response include:


  • Understand the correlation between price and quality, it will change your perspective


  • Identify alternative providers and steer employees to them


  • Take advantage of high deductibles by eliminating them to effectively steer to price and value


Rice University Healthcare Economist Vivan Ho On Price Transparency


Dr. Vivian Ho of Rice University is the James A. Baker III Institute Chair in Health Economics and Professor of Economics and an HBCH Advisory Board member. She recently was interviewed by Houston Public Media on price transparency. Her comments in this interview are worth the seven-minute listen. 


Key points include:


  • Most Houston hospitals are now posting something and provides specific examples for Houston’s dominant hospitals system


  • Insurance carrier posting is ahead of hospital posting and that is driving more hospital compliance


  • In 2023 prices must be posted for 300 shoppable services and in 2024 for all services


  • Employers will be able to use this information to negotiate better pricing
