Dear FVS Community,
 
We are very much looking forward to welcoming students back to FVS in August. As restful as summer vacation is, we miss the joy of a campus abuzz with the energy that our students bring, and we can’t wait to see their smiling faces. The last Back-to-School updates included information regarding August student arrival, as well as an email about required health forms. If you have not yet carefully read through the arrival plan, please do so as it contains important dates and community expectations regarding COVID-19 and our community.
 
Also, If you haven’t already, please Log-in to SchoolDoc.com to complete those necessary health forms. All required forms and consents must be complete BEFORE a student arrives on campus.
 
This email contains details about actions that must be taken before students can return to campus. All forms and required information must be completed by July 31. Students will not be permitted on campus unless this process is complete. Please read through this message thoroughly and feel free to reach out to the individuals designated at the end if you have questions or need help. I hope this letter will provide the majority of the information you need to prepare for the start of the school year, and please know that we are here to help in any way we can.

Student Permission Forms
This year, all Student Permission Forms must be completed by parents/guardians no later than July 31. These forms provide guidelines for the Deans’ Office when managing student excursions and other aspects of student life. The forms may be filled out by one parent, but must be signed by both parents. These forms are accessed by logging into the FVS website where users will be prompted to complete any forms required. If you need assistance, please email deansoffice@fvs.edu.

Health Forms
All families should have received a link to complete Health Forms through Schooldoc.com. This confidential portal allows families to share important information about their child’s health and to provide evidence of immunizations, consent for treatment, and permission/direction for administration of medication, if needed. Families may also share action plans for medical conditions such as asthma, seizures, or life-threatening allergic reactions, if applicable. A physical examination must be completed by a physician every 12 months. The physician will need to complete the Physical Exam Form below, which will also be uploaded through SchoolDoc.

If you need assistance or have questions related to health, please email nurses@fvs.edu. These forms must be completed by no later than July 31 and a student will not be permitted on campus if these forms are incomplete.
 
 

Athletics Forms- Completed by Parents
FVS will use BigTeams.com to collect and organize forms related to Athletics participation. All families, regardless of a student’s intention to participate in competitive sports, will need to upload a copy of their current year Physical Form (separately from Health Forms and SchoolDoc) and complete the CHSAA Form 1B, which is the Eligibility Form and Anti-Hazing Agreement. In addition, any student transferring from another high school must complete the CHSAA Transfer Form, which requires signatures from the athletic director and head of school/principal at the student’s previous high school. Look for an email in the coming week from the Athletic Department with further information.
 
Adviser & Roommate Surveys- Completed by New Students
Housing and advising placements are underway and we ask that new students complete the below forms as soon as possible. Day students do not need to complete the Residence Hall and Roommate Questionnaire. Housing and adviser placements will be available for students to view on the FVS Portal in early August and shortly thereafter, advisers will begin reaching out to their advisees before students begin arriving on campus.
 
 

Student Schedules & Ordering Books
Individual class schedules and information about book ordering will be available the week of July 26. Students will access their schedules by logging into fvs.edu. Please be on the lookout for a communication next week from Penny Steele, Academic Coordinator, once this information is available.
 

What To Bring
Please review this list to ensure that students are prepared with all that they will need. Upon arrival, if need be, FVS can help students acquire anything additional that they wish to purchase once in Colorado Springs. Please also note that unvaccinated boarding students must arrive with a week’s worth of clothing and supplies that they will need while in quarantine that is separate from the rest of their belongings.
