Over the last eighteen months we have faced challenges that no one could have ever foreseen or imagined. The staff, students, and school community worked together to be creative, flexible, patient, and strong, and we thank you. At the end of it all we all came out with a few bumps and bruises but have remained strong.That’s why our theme for the 2021-22SY is STRENGTH. Throughout next school year we will not only celebrate the strength our community exhibited over the last two years, but continue to remind and teach students the importance of remaining strong in challenging times.

MANDATORY FORM LINKS
DEADLINE AUGUST 6TH
With the new school year on the horizon, it is important that some procedural items are taken care of before the first day. Below are step-by-step instructions on how to complete the annual required paperwork for each student and family prior to starting the school year. Please read and complete each step.

You will find all of the necessary links below. For your convenience, the links are also located on the school's website. Go to the "For Parents" tab at the top of the home page, then click on the "Parent Resources" tab. 

The deadline for submitting all forms is August 6th.

 
Complete one entry for your entire family.


On this form, you will be filling out a two-page health information section for each child. It may be helpful to gather health insurance cards, doctor and dentist contact information, and emergency contact numbers before you begin.

*If your child requires an Individual Health Care Plan (due to chronic medical conditions), and/or a Food Allergy Action Plan (FAAP), please CLICK HERE to read an important message from Nurse Chouljian.

STEP 3 -- UPDATED RECORDS
 
Each student must have the items below on record prior to the first day of school. By Florida law, children are not allowed to enter school until these forms have been submitted and are on file.  
  • Updated immunization form (new and K-5 students only)
  • Updated physical form (new students and rising 6th grade students only)
  • Copy of the birth certificate (new students)

STEP 4 -- MANDATORY TECHNOLOGY ACCEPTABLE USAGE POLICY

Read the exciting announcement from the Technology department (found below) and complete and submit the Acceptable Usage Policy Form.

10 Easy Steps 
Google Takeout Instructions for students Leaving or Graduating from Park Maitland School
In order to transfer your child's data from your Google Drive when they leave or graduate from Park Maitland School, please follow these instructions before Monday, July 26th, 2021, as this is the date the school Google accounts will be suspended for non-returning students. These steps will allow you to download all the amazing work they created within Google Drive. Please do not directly share the school G Suite files with another personal Google account because when the school account is suspended, you will lose access to the files originally created on the school account. You can however follow the below process and then upload the files to a personal Google account once steps 1-10 have been completed. 

  1. Visit Google Takeout: https://takeout.google.com/settings/takeout on a laptop or desktop computer.
  2. Select the “Drive” icon
  3. Hit “Next” at the bottom
  4. Select file type “Zipped
  5. Select Archive size “2 GB
  6. Select “Send download link via email”- Please note that the archiving process could possibly take hours, depending on the amount of files. 
  7. When your archive is ready, you will receive an email confirmation to your school address @stu.parkmaitland.org or @parkmaitland.org. See picture below. 

9. Open the email and press “Download Archive” to each of the
blue tabs. 
10. Next, select where you would like the data to be downloaded
to. You can select the data to be sent to Downloads,
Desktop, My Documents, an external USB, etc. 

*As an additional and optional step, you can upload/drag your zipped Google Drive files onto a personal Gmail account once all previous steps have been completed.
2021-2022 Tuition charges will be added to your Alma account on August 1st.

* If on auto-draft, the charges will automatically be taken out on the first of every month on the third day.
