IMPACT Connect is a catalyst for change

Communities across the country have begun to realize the value of integrating data from multiple systems and using it to enhance their understanding of social determinants of health - factors which significantly influence a person's health and well-being.


Community Information Exchanges like IMPACT Connect improve real-time care coordination, reduce the use of costly public services, inform public policy, and support individuals’ progress in achieving quality of life indicators such as housing stability.


These changes are critical to improving population health, driving system efficiencies, reducing costs and advancing health equity.

We are currently building our network of providers at the Mezzo level and seeking partners in multiple sectors. Together we will enact real change at the Micro and Macro levels. Join us!
