Join us on Wednesday May 12, 2021 at 10am at 355 W. Dares Beach Road Prince Frederick, MD for a COVID 19 vaccine clinic. The Arc SoMD is partnering with The Pharmacy at St. Patricks to provide COVID-19 vaccinations to people with intellectual and/or developmental disabilities in the Southern Region, their caregivers that are 18 years old and over, direct support staff of people that receive services from the DDA, people in the Self-Directed Services delivery model, and Coordinators of Community Services.
If you are interested in being vaccinated, please bring your ID, insurance card, completed consent form and staff ID (if applicable), as well as a letter from your employer stating that you work with a person with a developmental and/or intellectual disability.
In order to have an exact count of the vaccination dosages that are needed, you will need to register for the vaccination. The deadline for registration is May 6, 2021.
If you are unable to come inside for vaccine, you can be vaccinated in your car. Please indicate that and an appointment time will be emailed back to you.
