Nursing Facility Transition program helps with discharge transitions

The Nursing Facility Transition program of Senior Resources is available to residents of nursing facilities who would like to return to their own home, move in with family, or choose another living option.

Transition navigators provide support and services after the move for those who meet health and financial guidelines. The transition navigators - Abbey Sachteleben, LLBSW, and Christine Yeomans-Searle, LMSW - will complete an assessment with the individual and anyone else they wish to have present. They will help identify barriers or concerns to a safe transition to a community setting, and offer options and referrals for community programs and services.

Senior Resources program revives the house call
Home Based Medical Care or “house call” concept is not new. In addition to enhancing the quality of care for patients and supporting caregivers, home-based medical care can significantly reduce medical emergencies, missed appointments, and the poor control of chronic conditions. Home-based medical care also helps alleviate stress and other challenges for everyone involved in a patient’s care.

Senior Resources' Enhanced Care Services provides a variety of services aimed at lowering hospitalizations. To qualify for the program, an older adult must be enrolled in Medicare, have at least one chronic health condition, have a qualifying insurance or private pay.

Become a Dementia Friend!

People with dementia sometimes need help going about their daily lives and to feel included in their communities. Dementia Friends works to give people an understanding of dementia, and the small things they can do to make a difference. 

Due to the pandemic, we are currently offering Dementia Friends via Zoom video-conferencing. All sessions are one hour.

To register for future sessions, email DementiaFriends@seniorresourceswmi.org with the date you would like to attend. A Zoom link and materials will be sent to you via email ahead of the session.

Free resources for seniors, caregivers
Active Daily Living, a free resource for older adults and caregivers, is available through the Senior Resources website. It is designed for seniors and/or family members concerned about the physical safety of a loved one. Interactive modules, videos, and weekly newsletters offer no-cost or low-cost tips developed by aging experts to help with activities of daily living.
