The First District PTA Commemorative Scholarship deadline of NOON, Monday, March 22nd is fast approaching. Here is some information to help you submit the applications you will be forwarding.
Students must use the First District PTA Commemorative Scholarship fillable forms and must answer all questions on the application and include a personal statement that touches on as many of the essay prompts as are applicable to the student. The applicant’s personal statement/essay should be typed and attached to the application. Students must include all required documents in the order prescribed in the application. Council PTA/PTSAs will review applications and select ONE student application from each high school PTA/PTSA in good standing in their council to be forwarded to First District PTA for consideration by the FDPTA scholarship committee. Applications submitted directly to First District PTA will not be considered.
Scholarship applications must be received in the First District PTA office by noon on Monday, March 22nd. You may submit the scholarship applications electronically to: officemanager@pta1.org or by mail to: First District PTA, 1008 S. Eighth St, Moor Field, Alhambra, CA 91801.
PLEASE NOTE: applications submitted by mail must be received (NOT postmarked) in the office no later than noon on March 22nd.
Applications submitted digitally should be in PDF form (preferred) but will also accept, if necessary, Word or Google Docs. Applications should be submitted in the order prescribed on the application.
RETURN APPLICATION PACKET IN THE FOLLOWING ORDER:
❑ Legible copy of your current PTA membership card on an 8 1/2” x 11” sheet of paper (PDF format, NOT jpeg or png) [2]
❑ Student Personal Statement – response to prompt questions (no more than 500 words total) PDF, Word, or Google doc only – should include word count. [3]
❑ Completed Reference Form with Letter of Recommendation – Filled out by School Faculty/Community Member [4]
❑ Copy of School Transcript [5]
If you submit the application digitally with each requirement as an individual file, please be sure to include the numbers shown above when naming the file. Also include student last name and council in the file name. Please help us be able to identify and include your students’ applications in the consideration process.
