Pfizer Vaccine is Now Available
at Jupiter Medical Center

Jupiter Medical Center has informed us that they will be receiving an additional supply of the Pfizer vaccine and is now accepting appointment requests for residents, who are 50 years old or older, for first dose vaccinations. To begin this process, go to JupiterMed.com, click on the gold banner at the top of the page, and complete the COVID-19 Vaccination Request to Schedule Form.
High risk residents between the ages 18-49 may also take advantage of this opportunity by having their Doctor submit an Electronic Physician Referral. If you are a high risk resident under the age of 40, contact your primary care physician and have them submit the referral form on your behalf.
Jupiter Medical Center will send an email notification and a text message reminder to eligible applicants within three (3) business days of receipt of either form.
