Dear Bloom at Bossier Associates, Residents & Families:
While the COVID-19 vaccine will be administered by CVS Health at Bloom communities, the Centers for Disease Control inadvertently marked Bloom's Louisiana community, Bloom at Bossier, as Walgreens (instead of CVS). The most efficient path forward is to schedule Bloom at Bossier with Walgreens, which will require you to download and complete the Walgreens' consent form below.
Our clinic will be scheduled for February 10, 2021 at 9:00am. We havent' received additional clinic dates yet. The Consent form is due by February 7, 2021 5:00pm to Laurie Manno: admin@bloomatbossier.com.
A more formal global Bloom vaccination clinic notice will go out in the next few days, but we wanted to provide you with the date and consent form for Bossier in the interim.
