Over the last decade, I can’t count the number of times I’ve heard this phrase and shared my patients’ sentiment. As a Mohs surgeon, I spend my days removing skin cancer from faces and putting those faces back together in a cosmetically pleasing fashion. I see firsthand how easily skin cancer can be treated for patients who are diagnosed early; conversely, I see the challenges people face when their diagnosis is delayed. Around one in five Oregonians will develop skin cancer in their lifetime. Dermatologists lead the way in helping everyone know how to identify it quickly and avoid the complications of delayed treatment.

Many people have heard of melanoma and justifiably so, as it is relatively common and can be deadly. The American Cancer Society estimates that 1,640 Oregonians will be diagnosed with melanoma in 2022 and 100 will die of the disease from spread to lymph nodes and organs. The good news is that, when melanoma is caught in the earliest stage, the cure rate from surgery alone approaches 100%.

Another form of cancer, basal cell carcinoma, is the most common type of skin cancer, with 3.6 million cases diagnosed in the United States each year. If you’re sitting down to play a game of bridge, someone at the table with you has probably had basal cell carcinoma. Basal cell carcinoma generally can spread to adjacent tissue and, if untreated, can form painful ulcers, disfigurement and loss of function of facial parts such as eyes, ears and noses. If neglected for a long time, it can eventually spread to the lymph nodes, other organs and even the brain. Basal cell carcinoma usually looks like a pink, pearly or translucent bump that slowly grows or doesn’t heal. The story I hear every day from my patients is that they thought they had a pimple, but it just wouldn’t heal. Many patients initially think their non-healing sore came from hitting their head on a car door or that their glasses were rubbing a specific spot, but after months of not healing, they eventually see a dermatologist. The good thing is that the vast majority of basal cell carcinoma can be treated with in-office surgery alone, but treatment when the cancer is small simplifies the process dramatically.

Yet another form of skin cancer, squamous cell carcinoma, has become increasingly common, with 1.8 million cases diagnosed in the United States per year. Although most squamous (rhymes with famous) cell carcinoma spreads only to adjacent tissue, it can also spread to lymph nodes and organs. Unfortunately, squamous cell carcinoma has become so common, the number of deaths from it is beginning to rival those from melanoma. Squamous cell carcinoma usually looks red, scaly and rough. It tends to occur in people who have had a lot of sun exposure over time. Just like melanoma and basal cell carcinoma, the earlier squamous cell carcinoma is diagnosed, the easier it is to treat, usually with in-office surgery alone.

If you have skin, it’s worth knowing what your spots are. I don’t know a single dermatologist who is offended by telling a patient that the spot they showed up for is benign and that there’s nothing to worry about. Similarly, we’re a pretty enthusiastic bunch about early diagnosis and treatment of skin cancer. It’s the gut-wrenching view of a spot that we may not be able to fix that makes us wish we’d met sooner. But even in those situations, we’re glad the person came in that day and not the next month.

The bottom line: If you have a spot that gives you any concern at all, have it checked out. You – and your doctor – will be glad you did.
