SMC Health is asking about your occupation, age, and if you have certain health conditions to help determine your eligibility for vaccination at different phases of the rollout, and to provide relevant information on how you can be vaccinated. They won’t share your personal information with anyone and will use what you tell them to improve planning and outreach. You will not be asked about immigration status.
Vaccine supply is very limited, so you may have to wait to get a vaccine even after you become eligible. The vaccine will be available to you for free.
