Announcing Agency Assessment Updates


Our HMIS is continuously making strides to streamline data entry and ensure that our HMIS is configured to optimally collect and record data to be able to accurately reflect and report on homelessness in our community.


In addition to the recent changes in the partnership agencies, assessments that have the primary function of connecting households to resources outside of “your” program and/or agency will no longer be available under the Partnership agencies. These assessments will need to be recorded under the HMIS End Users “home” agency.


  • All versions of the VISPDAT
  • Permanent Housing Assessment - Families (CA-503)
  • Foster Youth to Independence (FYI) Initiative Assessment (CA - 503)


If you are needing to conduct one of the above assessments with a Household, please note that you will need to be sure you are “working” under your “home” agency. To make sure of this, use the drop-down under your name on the top right hand of the HMIS home screen to select your default HMIS agency.

Helpful Case Management Tools for your Programs


If you're interested in enhancing your case management toolkit under the program assessment tab see below for a list of available tools. These assessments below can also be integrated into your program to streamline processes and provide comprehensive insights.


  • Individualized Support Plan (ISP): A comprehensive case management tool designed to guide individuals on their journey towards stability and self-improvement. It encompasses various essential elements regarding housing, community, and family goals.
  • Progress Review Form: Evaluates client status across housing vouchers, supportive services, and employment opportunities.
  • Rehousing Needs Assessment: It identifies the resources a household requires for stable housing. The tool assesses current and required services, a basic financial plan, and potential natural supports for the client.
  • Rental Unit Survey (Landlord Tracker): This tool gathers landlord information, property details, and rental availability. It can be used to enable efficient matching of individuals and families experiencing homelessness with suitable housing options.
  • Self-Sufficiency Matrix: This tool assesses an individual's independence and quality of life, aiding in service planning, progress tracking, and reporting to funders. It serves five key purposes: case management, client self-assessment, management guidance, measurement, and communication.
  • Housing Assessment Plan (HAP): Collects vital details on client household, income sources, and monthly budget to chart a housing plan.


You can find these case management tools under program assessments in each program enrollment. The HMIS team also has the availability to build customized tools to fit your program's needs.
