NEW ANNUITY POLICY RECEIPT FORM – HF158
EFFECTIVE IMMEDIATELY
Please use the HF158 in order to document the date when an annuity contract is delivered to a client (Located in the Harbor Forms Library). Documenting the date of delivery provides details on when the client’s free look period begins and ends.    In the event the Insurance Company forwarded the annuity contract directly to the client, you will need to reach out to the Company to determine the date of receipt of contract. 

Please note that a client signature is required and may be forwarded to the client through DocuSign. If the client wet signs the document vs. submitting through DocuSign, the signed document should be routed to [email protected].

Please note that failure to obtain this required documentation may result in delay of commission payout(s) related to the annuity contract.
