This April we are bringing awareness to National Child Abuse Prevention Month!


We know that community health stabilizes and improves when individuals have connection to systems and environments that support them. For this reason, Sonoma Connect | Sonoma Unidos is committed to supporting and building a network with organizational capacity that supports a thriving workforce and community self-efficacy. We are excited to be on this journey with you to advance equity throughout our Sonoma County system of care, to ensure the safety and well-being of children and youth across the county.

Announcements

On Tuesday, March 22, Sonoma Connect | Sonoma Unido’s Community Leadership Board voted to ratify the final vendor selection for the Medical and Social Service integration platform to support closed-loop resource referrals. The selection has been narrowed down to three vendors that best represented community input on desired functions.


After careful consideration, the Community Leadership Board ratified NinePatch as the final selection.

Next Steps


SC|SU will partner with RCHC and engaged workgroup members to complete next steps now that the technology vendor has been identified to:


  • Articulate referral workflows and roles and responsibilities of users. 
  • Develop levels of access based on consent from clients and agreements with organizations that promote case management that is shared between organizations, ensuring the client at the center. 
  • Define what training is required to meet adopted goals and standards. 
  • Design criteria for how the resource and referral system will be implemented. 
  • Define what success for the resource and referral system looks like, how success will be measured, and track progress. 
  • Create and adopt a five-year operational plan that also includes a plan to pay for and sustain the system for the long term.

ACEs Clinical Action Team 

Thursday, April 7, 2:00pm-3:00pm 

 This month the ACEs Clinical Action Team will review results of the round three trauma-informed network of care assessment.  


Neighborhood Resource Centers Action Team 

Wednesday, April 13, 3:00pm-4:30pm 

The NRC Action Team will discuss next steps to gathering CHW/P insights into how to advance and support the CHW workforce pipeline development.  


Community Accountability Action Team 

Thursday, April 21, 1:30m-3:00pm 

This month, CAAT will continue its conversation on DEB practices, assess our capacity and identify areas of opportunity to support the transformation of community-led systems change. We will also be discussing data equity frameworks and further developing our strategy for community engagement.  


Resource Connection Network Action Team  

Monday, April 25, 1:00pm-2:30pm 

Join us to discuss and plan for next steps for build-out of NinePatch. We will also discuss starting a Data Governance workgroup to move forward work on data policies and procedures and data sharing agreements.

Spotlight on our ACEs Clinical Action Team Co-chair: Dr. Deirdre Bernard-Pearl!

Dr. Deirdre Bernard-Pearl is the Director of Pediatrics at the Santa Rosa Family Medicine Residency at Santa Rosa Community Health. She has a long-standing interest in equity and trauma-informed care, and was a pioneer in ACEs screening in primary care. Dr. Bernard-Pearl serves on the California Surgeon General’s Trauma-Informed Primary Care Implementation Advisory Committee.
