Has the Supply Chain Failed in the Covid-19 Vaccine Launch?

The media reports on the rates of Covid-19 vaccination make one think that the supply chain has failed in getting vaccines to people who want and need it. There are two approvals of Covid-19 vaccines (Pfizer / BioNTech and Moderna). Other Covid-19 vaccines are on the way to approval.
Phase III clinical trials reveal that there are vaccine candidates ready for approval by the United States Food & Drug Administration (FDA), including AstraZeneca / Oxford, Janssen (a Johnson & Johnson company), Novovax, and others in the US. This includes one in Germany, one in India, one in the United Kingdom, and none in France. Has the supply chain really failed at delivery?
