Profound racial and ethnic disparities in health and well-being have long been the norm in the United States. Black and American Indian/Alaska Native (AIAN) people live fewer years, on average, than white people. They are also more likely to die from treatable conditions; more likely to die during or after pregnancy and to suffer serious pregnancy-related complications; and more likely to lose children in infancy. Black and AIAN people are also at higher risk for many chronic health conditions, from diabetes to hypertension.
Care transitions that involve moving from one health care provider to another can often be stressful and daunting for patients; looking for high quality follow-up care while trying to manage costs can be complex. This process is equally cumbersome for nurses and care coordinators who want to ensure that they send their patients to the best in-network providers. Furthermore, the lack of a central and transparent process lends to the difficulty.
Several weeks ago, we launched a survey to determine the post-acute growth strategies that owners, operators, and managers would be focused on in the new year. Given the impacts COVID-19 has had on the post-acute industry, it was clear to us here at Trella that traditional, linear growth wasn’t going to be the focus for every organization or line of business. Instead, post-acute ACO (or DCE) partnerships will also be a top priority. 
Health systems are planning to advance their risk-based payment strategies by taking on more upside or downside risk, professional capitation, or global capitation in Medicare Advantage lines of business in 2022, according to a recent survey. The executive survey conducted by the Healthcare Financial Management Association (HFMA) for the Guidehouse Center for Health Insights found that nearly 60 precent of health systems are looking to advance into risk-based Medicare Advantage models in the next year.
