Position: Client Care Coordinator

  • This is a full time position, M-F 8:30 am to 4:30 pm in Long Branch Monmouth County. The Client Care Coordinator (CCC) is responsible processes incoming requests for service and coordinates and schedules Supportive Aides and Certified Home Health Aide assignments under the direction of the Home Care Supervisor and in collaboration with the Director of Nursing. The Client Care Coordinator is responsible for ensuring that all time sheets reflecting aide activity are received in a timely manner, and that all subcontractors comply with time sheet submission requirements pursuant to Contract. In order to be successful you will need at least two years of home care management or administrative experience including scheduling. You must have high energy, outgoing personality and be comfortable with speaking on the phone and be a team player. High School or some college preferred. FCS is the oldest social service non-profit agency in Monmouth County with 111 years of experience.
 


Position: Certified and Noncertified Home Health Aides

Family & Children's Service(FCS) is a social service agency with 13 programs and services dedicated to improving the lives of the vulnerable in the community. We're hiring for Certified and noncertified home health aide positions to help fulfill our mission. Certified Aides provide personal care and assist with client’s basic needs and are certified with the NJ Board of Nursing. Supportive Aides do not require an NJ license and will assist with light housekeeping and grocery shopping. Please have an updated physical. We offer NJ Sick Time and a flexible schedule.

Open Position: Registered Nurse Field Supervisor with ADON Duties

The Registered Nurse Field Supervisor is responsible for general case management of caseload. Maintains accreditation compliance home visits, phone calls, and supervision of aides. Requirements: Must have two years’ experience in professional nursing practice. Two years of Community/Home Care Nursing preferred. Current unencumbered NJ Professional Nurse License including completion of mandated continuing education requirements.

Open Position: Adult Protective Services Social Worker
 Details: Full time, M-F 8:30-4:30, Long Branch
 To render direct social work service to a selected group of clients in a manner to insure maximum benefits to clients. To perform all related activities, such as (1) making full use of the agency’s resources and outside facilities, (2) performing basic administrative tasks, and (3) participating in research and community work.
Requirements: Must have a Bachelor’s Degree in Social Work; Two years’ experience in protective services or social work; Must possess a valid New Jersey’s driver’s license and an available vehicle.Experience in geriatric social work preferred.

Open Position: Adult Protective Services Intake Coordinator

Details: Full time, M-F 8:30-4:30, Long Branch and Middlesex
Responsible for processing incoming referrals, completing collateral contacts for referrals, and conferencing the referrals with the social work team to determine if referrals are to be screened in or out for the Adult Protective Services (APS) program. This is a grant-based position and can be subject to termination should the grant be discontinued by the county/state funder. Requirements: Associate degree in a health related or behavioral science field required, Preferably one year of experience working with the elderly or physically disabled in an institutional or community setting, Bilingual Spanish/English preferred.
