Dear Roslyn Families,

We are writing to update you with further guidance received from the New York State Department of Health. Effective immediately, the following completed forms must be printed and provided to the school nurse upon your child’s return from a COVID-19 quarantine or isolation only. They may be emailed or dropped off in person, whichever is more convenient for you.

Complete these two forms if your child has tested positive for COVID-19 and has been in isolation:

• Affirmation of Isolation 
• Post COVID-19 Sport and Physical Activity Clearance Form This form must be completed in order for your child to be cleared to participate in all school sports and physical education classes/activities.

Complete this form if your child has been identified as a close contact to a COVID-19 positive person: 

• Affirmation of Quarantine
