A Message from the CEO
I am very excited to announce the official opening of our 4th location in Barto, located at 1315 Route 100 North, Barto, PA. Currently we are providing face to face and telehealth medical visits as well as telehealth behavioral health visits. We are working on finalizing the set-up of our 2 dental chairs within the Barto location with the intent of having them open by the end of this year or the beginning of the new year. At this time, any patient needing a dental appointment can contact our main number at 610-326-9460 and schedule a dental appointment at our other locations. Please remember we provide free transportation to and from all CHDC locations- please check out our website www.ch-dc.org for a list of pick up and drop off locations and times.

Another friendly reminder to everyone to get their flu shot as it is more important than ever right now with the pandemic still in place. CHDC continues to provide a safe and clean environment as we are following social distance practicing, screening questions and mandatory mask coverings to ensure your visit to CHDC is up to your expectations.

CHDC is devoting time and effort to work with our staff to improve the patient experience. It is important to our organization each and every person coming to CHDC has the best patient experience. We will be providing an on-going patient experience monthly training beginning in November with our staff to continue to improve your patient experience at CHDC. If you happen to come in for a visit, please tell us what you think by taking a quick survey on our survey kiosks located throughout our locations as every opinion counts!

Finding Family Balance: Navigating through Stress

Eat Fresh Fall Program Updates

After a successful summer season, in partnership with Chester County Food Bank Eat Fresh Program, the Care Management Team Kicked off the Eat Fresh Fall program at CHDC. Care Management distributes the boxes of fresh produce to Community Health & Dental Care patients and their families that are experiencing food insecurity. The team distributed over 150 boxes to families over the summer and are set to increase those numbers this fall. The Care Management Team coordinates box pickups and deliveries to the home. The Eat Fresh program allows participants to gain positive attitudes toward a variety of nutritious foods including fresh fruits, vegetables, and whole grains. The program offers six weekly bilingual virtual sessions provide hands-on cooking opportunities and encourage individuals to learn more about their food environment and acquire basic knowledge of nutrition. With these skills and the opportunity to taste new fresh fruits and vegetables, participants will be able to make informed choices to select ingredients and prepare healthy meals at home.
The Care Management Team also supports the Fresh to You truck through the FVRX programs and the Veggie Bucks programs. The Fresh to You truck is open to the public and is located alongside the CVS lot at the Coventry Mall on Tuesdays between 4:00 pm – 6:00 pm.

CHDC is excited to participate in The Amazing Raise to raise funds in support of CHDC patients who struggle with food insecurity.

CHDC’s care management team identified 170 patients throughout May and June 2020 – about 10% of our patient population – with food insecurities. With limited grant funding approved to be re-allocated towards helping these patients, CHDC purchased grocery store gift cards and distributed them to these individuals to help put food on their table. In only two short months, our $12,000 grant fund was depleted.

As the pandemic continues, CHDC recognizes the ongoing and growing need of our patients – especially as we head into the colder months. To replenish our food insecurities fund, we made the decision to opt-in and participate in our local Pottstown community’s one day donor match campaign called The Amazing Raise. For every dollar YOU donate through The Amazing Raise on October 30, 2020, CHDC will receive an additional percentage in dollar match from generous Amazing Raise sponsors, stretching your contribution even farther!  Not only will enable us to purchase gift cards from area grocery stores, but you will be part of the compassionate solution in helping our patients like M.P. and T.T. above.

Keeping our Patients Safe
At Community Health and Dental Care, we are committed to a safe and clean environment as the COVID-19 pandemic continues. Locations have been triple-cleaned, all staff are wearing masks, and seating in lobby and waiting rooms are socially distant.

We want to send a special thank you to Transcend Facility Management Company, LLC for all they do to ensure our CHDC facilities are clean and safe. Check out this cool video featuring our main office at the Coventry Mall.
Octodent equipment have been installed in our dental facilities, and patients are being scheduled to allow for appropriate levels of social distancing in our facility. COVID-19 pre-screening includes temperature checks of all patients upon arrival for their scheduled appointment. All patients and staff are required to wear masks.

The Tri-County Health Council has launched a multi-faceted campaign effort to reach members of the Greater Pottstown region to encourage people to seek immediate emergency care if experiencing signs of a stroke or heart attack. Many people are experiencing anxiety about going to the hospital for emergency care due to the pandemic. However, our hospitals and healthcare facilities are fully equipped and safe for people who need immediate care.

Help us spread the word and reduce the harm done to individuals who might ignore symptoms of stroke and heart attack and not seek care.
