District 65 and District 67 Offer Vaccine Clinics for Students
5-11 Years of Age
To Our District 65 and District 67 Parents and Guardians:

On Tuesday, November 2, the CDC announced its endorsement of the CDC Advisory Committee on Immunization Practices’ (ACIP) recommendation that children 5 to 11-years-old be vaccinated against COVID-19 with the Pfizer-BioNTech pediatric vaccine

Two Vaccine Clinics
In an effort to provide our community members with this option, Lake Bluff School District 65 and Lake Forest School District 67 are partnering with the City of Lake Forest and Village of Lake Bluff to offer two voluntary COVID-19 vaccine clinics free of charge for students 5-11 years of age. 

Following a survey of parents and guardians of 5-11-year-olds in both districts, approximately ⅔ expressed interest in such an offering. Roughly 40% of those who expressed interest noted that a weekend date would be preferable, while about 60% preferred a weekday. With this in mind, we are offering the following to all families with 5-11-year-olds in our communities:

Registration
Vaccines will be administered free of charge by Passport Health. Each participant needs an appointment. Walk-ins will not be accepted. Here is the link to register. This link will allow parents and guardians to create an appointment and complete their child's electronic medical consent form. A parent or guardian must provide written consent and be present for a child to receive the vaccine. Your 5-11 vaccine registration checklist may be found here
 
Following the first dose vaccination, participants will move to an observation area where staff will be available to schedule appointments for the second dose.
