Awareness Surveys for Accessing Primary Healthcare as an Individual With Intellectual or Developmental Disabilities

We are sharing these surveys on behalf of the Idaho Council on Developmental Disabilities. Please feel free to complete the survey that applies to you or share with others. Thank you!

The National Association of Councils on Developmental Disabilities as part of our partnership with United Healthcare is working on a small project to help us understand the experience of individuals with intellectual or developmental
disabilities (I/DD) or similar functional disabilities in accessing primary healthcare services.

Our goal is to generate awareness for individuals, caregivers, and family members on the benefits of primary health care and to provide connections to resources for healthcare providers.  

Their first step is launching a series of surveys to leaders in the DD community, individuals and caregivers, and health care providers. 

Advocate / Individuals Survey:

This survey is intended for advocates and individuals, including family members and caregivers, and can be used outside of the NACDD community

Understanding Idaho Medicaid

Join Idaho Parents Unlimited as we help to demystify Idaho's Medicaid!

Learn where to start, about children with special healthcare needs and disabilities, mental health options for children and families, and more!

This will again consist of a normal, hour long training that will be recorded, followed by a half-hour question and answer section that will not.
