One Week Left to Meet Public Health Order Compliance Requirements
|
|
|
|
About 73% of our team members have submitted documentation in compliance with the city’s Public Health Order issued on Aug. 2. As a reminder, team members who have yet to take action have until Sept. 30 to upload their documents into Employee Space. Please submit your documentation today and avoid disciplinary actions beginning in October.
There are three ways to meet compliance requirements:
-
Submit Documentation of Vaccine Completion: Upload a picture of your completed vaccination card using these instructions. Note: Only JPEG and PDF file types are allowed in the system.
-
Lost your vaccine card? Click here to learn how to obtain your COVID-19 vaccine record. If you got your vaccination in Colorado, you can also download the myColorado App and click on the myVaccine Record section. The app may require additional login information to access the record the first time.
-
Submit a request for a medical exemption: You and your medical provider will need to complete this form prior to uploading to Employee Space. Follow these instructions to upload your documentation.
-
Submit a request for a religious exemption: Complete this form to request religious exemption. Please note that all sections of the form must be completed, including a description of the sincerely held belief. Follow these instructions to upload your documentation.
For more information on COVID-19, the vaccines, or the public health order, visit the Vaccine Information page on The Commons.

5. “When I’m feeling blue or stressed out during fall, I enjoy driving up through Independence Pass and hiking the Devil's Punch Bowl or Maroon Bells to see the colors change.”
-- Joel Vasquez, Ellis

4. “One of my favorite fall activities is heading to Anderson Farms in Erie. I love taking the tractor out to the pumpkin fields to pick out a pumpkin to carve with my niece and nephew. We go through the corn maze to find out which farm animal committed the silly crime! Then, we get some funnel cakes and apple cider, and take photos all around the farm.” -- Anthony Bryant, Inspire

3. “My favorite part about fall is marching band season! Watching my daughter have the same great experiences I had while I was in the Denver Citywide Marching Band is incredible. Although she is in Jeffco, the memories she is making will last a lifetime. This photo is of the Denver Citywide Marching Band at the Tournament of Roses Parade in Pasadena, California on Jan. 1, 1997.” -- Melissa Davis, Abraham Lincoln

2. “My favorite part of fall is ... all of it! This year, I have been going all-out with the pumpkin-flavored coffee, the pumpkin-scented candles, and I'm even thrifting for some fun and budget-friendly fall decor! My favorite of all is that with Halloween around the corner, it's almost time to celebrate black cat season. My cat is excited about all things spooky.” -- Alicia Roman, Swansea

Cristina Simental-Alvarado
Kindergarten ELA-S, MSLA
Students First
"Cristina embodies the value of Students First by making her room student-centered and focused. At the beginning of the year, Cristina had her room set up and arranged, but after hearing the idea of a 'calm down' corner from a peer, she shifted spaces to ensure that she could include this in her classroom. Additionally, rather than decorating the class her way, Cristina included students' voices and student-created displays so that the students feel seen and heard in their room. Cristina is always striving to put Students First in all that she does."
-- Hannah Gonzalez, Mathematics and Science Leadership Academy
