The Essential Elements of Home-Based Primary Care™ workshop is designed for learners considering, or relatively new to, offering home-based primary care (HBPC) services for patients with complex medical conditions. 

The workshop features case-based sessions focusing on clinical and economic factors unique to HBPC, including patient assessment; infection control; medication management; acute/urgent care; technology useful in enhancing patient care and productivity; cultural competence; staffing, operations and durable medical equipment; coding and billing; provider safety and self-care.

Advanced Applications of Home-Based Primary Care™ workshop

The Advanced Applications of Home-Based Primary Care™ workshop is designed for experienced home-based primary care (HBPC) providers and practice staff.

Case-based sessions focus on higher-level topics in HBPC such as advance care planning, prognostication, care coordination and transitions of care, caregiver support, and challenging clinical issues such as dementia and polypharmacy. In addition, this workshop features tips and strategies related to coding and documentation, workflow and staff productivity, and demonstrating the value of HBPC.

On Wednesday, May 20, HCCI hosted an HCCIntelligence™ webinar entitled,  Putting Person-Centered Decision Making into Practice . During the webinar, participants learned how to improve their ability to make clinical decisions that align with, and honor, an individual’s goals, values, preferences and decisions. The presenters also walked through how to develop strategies that create an inter-professional system to ensure person-centered decisions are made. Survey on Understanding COVID-19
in Home-Based Care Practices
We recently shared a survey on behalf of our colleagues Drs. Christine Ritchie and Bruce Leff and the National Home-Based Primary Care Learning Network. The survey’s purpose is to increase the understanding of COVID-related challenges that home-based practices are facing and their approach to resolving them. The survey is also intended to help these practices learn together how to address pandemics. This one-time survey can be completed by anyone in your practice and should take 10 minutes or less to complete. To participate,  click here to complete the survey by Tuesday, June 9th.
