ADD YOUR VACCINE CARD TO YOUR MEDICAL CHART
Got your COVID19 vaccine? Here’s what to do next!

If you've received your COVID19 vaccine, your provider would like to know so we can update your medical history. Just like vaccines for Mumps, Tetanus, or Flu, the details of your COVID19 vaccine should be part of your medical chart. It ensures documentation will stay with your history no matter where you receive your care. That's why we're requesting a copy of your COVID19 vaccine card.

It's easy to add your vaccine card to your medical records
You can choose from one of the following easy methods:
  • STOP BY any ACHS site and provide your vaccine card to our Patient Access Specialist, they'll make a copy and add it to your chart. Plus, when you stop by with your card – you’ll get a FREE gift to thank you for your time!
  • EMAIL a photo or PDF copy of your vaccine card by to: mdewoodsville@achs-inc.org
  • UPLOAD a photo or PDF copy of your vaccine card through your ACHS patient portal Don’t have an ACHS Portal account? Call your local ACHS site!
  • FAX a copy of your vaccine card to 603-444-8133
  • MAIL a copy of your vaccine card to ACHS-Woodsville -
  • 79 Swiftwater Road, Woodsville, NH 03785 - Attn: Medical Information Specialist

Ready to get your COVID19 vaccine? ACHS can help.
The COVID19 vaccine can help protect you from getting COVID and may decrease your symptoms should you come down with the virus. Once fully vaccinated you can feel more comfortable spending time with loved ones or going out in public. If you are ready to get your vaccine, ACHS has upcoming vaccine clinics where you can choose from the Moderna or Johnson & Johnson vaccine. You can register at nh.gov, 2-1-1, or call any ACHS site and ask to speak to an ACHS patient navigator.

As we all begin to resume our “normal” activities, such as shopping, going out to eat, and seeing friends, it’s important to remember that until the majority of our population is fully vaccinated, we should continue to follow CDC guidelines. For the near future, ACHS will continue to require masks for staff, patients and visitors.
