ILHITREC Rolls Out Advanced Alternative Payment Model for Illinois Primary Care
ILHITREC is rolling out an Advanced Alternative Payment Model (AAPM) for primary care providers, PAs, and NPs in Illinois. The CMS Primary Care AAPM is a Direct Contracting advanced alternative payment opportunity that focuses on reinvesting dollars back into primary care. This program leverages the Clover Assistant Tool, a web-based application powered by clinical and claims data on Medicare beneficiaries. Clover Assistant dynamically surfaces up-to-date, patient-specific information, including care gaps, medications, and potential diagnoses, providing you with a comprehensive view, populated with data from anywhere aligned beneficiaries have received care. Minimally, participants need only confirm diagnosis on 90% of Medicare FFS beneficiaries.

Participation in the CMS Primary Care AAPM provides primary care clinicians with many benefits:

Better Care
  • Access 3 years of claims data at point of care on your Medicare FFS beneficiaries
  • Customized analytics by provider on key Population Health metrics
  • Optimize hierarchical condition coding (HCC) to ensure capture of all chronic conditions, a frequently missed opportunity for additional revenue that can be earned per visit for your complex patients.
Financial Benefits
  • earn at least 30% more revenue per visit on aligned Medicare FFS beneficiaries.
  • earn additional revenue by receiving a $3 - $5 per member, per month (PMPM) fee
  • additional revenue is paid with claims rather than 1 ½ years later like many shared savings programs
  • select from No Risk options
  • receive 5% AAPM MIPS bonus
  • participants are excluded from MIPS reporting
  • receive direct support, on-site and remote, from ILHITREC Quality Improvement Advisors

Earn your last payment in the 
Illinois Medicaid PI Program!
We have entered the LAST Attestation Year for the Illinois Medicaid PI Program. The attestation period for the 2021 program year started May 1, 2021 and ends August 31, 2021. The latest possible 90-day EHR reporting period is June 2, 2021 through August 30, 2021.

Medicaid Provider Incentive Payments states that no payment may be made after CY2021. It is for this reason that the attestation periods for the final program year have been condensed. We encourage you to attest early since this is the end of the program and there will not be any extensions.

Looking for updated 
Direct Addresses for 
your referral partners?

CMS has a weekly incremental NPI file with direct addresses and a full replacement monthly NPI file that includes deactivated NPIs.  

These files are free to download and can be found at NPI Files (cms.gov). If a provider has not updated their direct address in NPPES it will not appear in the files. This file can be a good way to double check if your provider's direct address is entered correctly or you may be able to upload this file to your EHR to update your directory of referral partners.
