Update on COVID emergency funding Call for Proposals issued by the CHRC
 
The COVID-19 Virus Emergency Relief Call for Proposals issued by the CHRC last month generated a total of 63 letters of request for funds totaling $2.6 million. The Call for Proposals is supported by federal funding received by the state and provided by the Maryland Department of Health.
 
The COVID-19 Call for Proposals is designed to provide immediate and timely financial support for Maryland's safety net providers, many of whom are on the front lines of delivering services for vulnerable residents and are responding to the impact of COVID-19.
 
Of the 63 applicants who submitted letters of request, 40 are first-time applicants and 54 have not received a grant from the CHRC in the past. Click here for a list of applicants who submitted letters of request.
 
Following a review period, COVID grant awards are anticipated to be made by the CHRC Commissioners at the Commission's next meeting in late June. Click here for more information about the CHRC's COVID Call for Proposals.

Mt. Laurel receives grant from CHRC to support chronic disease management program focusing on diabetic management
 
Mt. Laurel Medical Center, a federally qualified health center in western Maryland, received a two-year grant for $195,000 to support an expanded case management program that will address the needs of its diabetic patients. The diabetes care management grant complements Mt. Laurel's existing array of services that includes primary care, integrated behavioral health, pharmacy and nurse care coordination. The CHRC grant is projected to serve approximately 30 individuals over the course of the grant. For more information about this grant, click here.
 
The chronic disease management grant is the second grant awarded to Mt. Laurel by the CHRC. The first grant, awarded in 2016, provided dental services for 249 individuals.
 
The chronic disease management grant to Mt. Laurel is one of 12 grants totaling $2.2 million by the CHRC this year to support programs that will address food insecurity, a key social determinant of health, and will help support implementation of the Maryland Diabetes Action Plan. The Maryland Primary Care Program (MDPCP) Announces the Release of the 2021 Request for Applications (RFA)

The Program Management Office (PMO) within MDH's Public Health Services recently announced that  CMS has released this year's Request for Applications (RFA) for 2021 participation in the Maryland Primary Care Program (MDPCP). The application portal will be open for new practices and Care Transformation Organizations (CTOs) to apply to participate in January 2021. The deadline for applications is July 14, 2020. The Maryland Opioid Operational Command Center (OOCC) Announces Grants Application Deadline Extension.

The OOCC is further extending the deadline for the FY2021 Competitive Grant & Block Grant programs due to the coronavirus disruption. Applications were due on May 29, 2020.
