Vaccine appointment scheduling through Kaiser Permanente

Dear NLACRC community,
 
Kaiser Permanente is now scheduling vaccine appointments for Regional Center consumers age 16 and over by calling 833-KP4-CARE (833-574-2273) or through www.kp.org.
 
Kaiser is able to schedule vaccine appointments for both Kaiser members AND non-members. There is no charge to receive vaccine through Kaiser.
 
You should bring a photo ID with birthdate as well as your NLACRC vaccine authorization letter to your appointment.
 
Please note that Kaiser advises that parents of 16 and 17 year old consumers directly call 833-KP4-CARE to schedule their appointment. Currently, only the Pfizer vaccine is authorized for 16 and 17 year olds, and calling 833-KP4-CARE will help ensure that you are scheduled at a clinic providing a Pfizer vaccine.
