Welcome to Mercy! We hope you are enjoying summer and look forward to starting a new school year. Please review the important information below in regard to FinalForms, Physical Requirements, Fall Sports, Transportation, Uniforms and iPad Lease Agreement.

Please note the 2021-22 student schedules will be posted on PowerSchool in early August. All students and families must be sure all required forms are submitted in FinalForms, physical requirements are completed and student billing account is current in order to access Mercy email account, course schedules and book purchasing.

FINALFORMS STUDENT INFO ONLINE MANAGEMENT
Mercy High School utilizes FinalForms, an online forms and data management service for student and parent/guardian information. 

FinalForms allows you to electronically complete and sign enrollment, back-to-school, and athletic participation forms for your daughter(s). FinalForms saves data from season-to-season and year-to-year so that you will not need to enter the same information twice. FinalForms also pre-populates information wherever possible, saving you time.

All students are given a Mercy email address (@mhsmi.org), and will need to sign-in to the account to receive FinalForms information. Please click here for new student email instructions.

IMPORTANT: You will receive an account confirmation email from FinalForms before July 7th. From this email, please click the registration link and follow the steps to login and access your student(s). 


Once you confirm your account in FinalForms, you will be able to review and edit your data at any time. Each school year, you will be required to sign new forms and verify accuracy of your information.

If you do not receive an email from Final Forms by July 8, 2021, please check your spam/bulk email folder and contact Kate Scalzi at kmscalzi@mhsmi.org.

We kindly ask all parents to please complete the FinalForms process for your daughter(s) before August 3, 2021. Thank you for your support and we hope FinalForms is an easy-to-use online tool for managing your student's information. 
PHYSICAL REQUIREMENTS & FALL SPORTS INFO
Physical Requirements
All incoming 9th graders, transfer students, athletes, and students taking a Physical Education class, will need a current physical, and complete the FinalForms Profile

For the 2021-22 school year, you will need a physical performed on/after 4/15/2021.

Fall 2021 Athlete Info
All Athletes are required to have completed the FinalForms Profile. Registration for Fall 2021 is required to try-out and students should complete survey here by August 3. Please select all sports in which you are interested in FinalForms, as well as completing registration survey.

Try-out times will be on the Athletic Calendar and team specific pages as available. Please be prepared to attend all try-out sessions. Fall sports (Cross Country, Equestrian, Field Hockey, Golf, Swim & Dive, Volleyball) start date is tentatively August 9, 2021. Please stay tuned for specific sport team updates the week of August 2.
TRANSPORTATION
Bus Service
The bus transportation information, schedule and passes for the upcoming 2021-22 school year are available here. Passes are available for purchase as a full year, semester/season or 10-ride occasional. Visit mhsmi.org/transportation or contact Kate Scalzi at kmscalzi@mhsmi.org with any questions.

Carpool Directory
The directory for Mercy HS carpool interest is now available. Click here to view the 2021-22 Mercy student list for forming carpools. This google spreadsheet is a live document and will be continually updated as needed. Email admissions@mhsmi.org if you have any changes or do not wish to be included. Please note this is not a full student directory, as some families requested to opt out of this carpool list. 

Student Parking Pass
All student drivers are kindly asked to complete the online student parking pass request form here for the 2021-22 school year.
UNIFORMS
Each class wears the same color kilt for two years. Freshmen and sophomores wear plaid patterns, juniors and seniors wear solid colors. Mercy uniform attire is available for purchase exclusively from Educational Outfitters. All grades wear banded white or maroon polo tops (short or long sleeve), banded white blouses (short or long sleeve), and black sweaters (cardigan, v-neck or vest).

Everyone is encouraged to go to Educational Outfitters for fitting and purchasing. Only black sweaters purchased from Educational Outfitters, black quarter-zips purchased from the Marlin Shop and Mercy Varsity letter sweaters may be worn in addition to the required uniform blouse or polo and kilt (not in place of the blouse or polo).

Please note kilts should be worn no more than two inches above the knee. Uniform skirt lengths will be checked the first full week of school. Any student who permanently shortens her uniform skirt to an unacceptable length will be required to purchase a new skirt.
iPAD LEASE AGREEMENT CHANGES
Please note the following changes to the iPad Lease Agreement. (Changes are listed in bold italics.)

Equipment Issued - You will be leasing an Apple iPad (current year model), Charger (Power Cord) and case. MHS retains sole right of possession of the iPad and related equipment until the lease term is completed or a buyout occurs. The iPad will be issued to students according to the guidelines set forth in this document. Your Apple iPad is for educational use and will be maintained, managed, and monitored while on the school network by Mercy High School. 

4 Year Warranty - Bundled with your lease is a 4-year AppleCare+ warranty for the iPad. The warranty extends the manufactured iPad coverage for hardware defects or malfunctions for the four years of the lease. Additionally, this warranty covers accidental damage over the four year warranty period, covering two incidents per year at no charge. Parents and students must report any damage to the MHS IT Department. The warranty does not cover a lost or stolen iPad. What is Covered? Accidental Damage, cracked screen, damaged ports, manufacturer’s defect, iPad (Ports, buttons, sensors, screen), charger (Not caused by misuse or loss), battery capacity of less than 50%. What is not Covered? Misuse or intentional damage, Natural disasters, Wear and tear, Apple Pencil, or case.
