Dear St. Vincent de Paul Faculty & Staff,
 
St Vincent de Paul High School will continue to provide a COVID testing program in 2021 for the SVES & SVHS communities. We will be offering this testing opportunity in partnership with a new vendor PMH Laboratory.
 
Our next testing day will be Wednesday, February 10th.
 
COVID Testing at St Vincent's High School for SVHS & SVES Communities

  • When: February 10th  9:00am - 5:00pm
  • Where: St. Vincent de Paul High School parking lot between the athletic field and the gymnasium
  • Who: All faculty, staff, coaches, students, parishioners & families 
  • Parking: In front of the DeCarli Gym parking lot

Information Snapshot:
  • There is NO FEE for the test.  
  • While there is no fee for the test, proof of insurance/or lack of insurance is required. Regardless of one's insurance circumstances, no one is denied the opportunity to test.
  • The entire process from check-in to completion will take approximately 5-7 minutes. 
  • We cannot guarantee an appointment to everyone. 
  • You will receive a notification email within 48 hours notifying you that your results are ready.

  • Appointments will close on Feb 8th. Once the scheduling calendar closes, we will try to accommodate fill in appointments as cancellations are made.  
 
Important Reminders:
  • Your mask must be on when you leave your car.
  • Please arrive ten minutes before your appointment time. You will be greeted by staff for check-in and paperwork intake.
  • Students will NOT be allowed on campus to make copies of or print out their necessary documents.
 
What To Bring With You:
  • Photocopy of your insurance ID card - front and back.
  • Photocopy of a government ID card (driver's license, ID Card, Passport, etc)
  • If you are uninsured, please complete this FORM.  
  • Please complete this RT-PCR CONSENT FORM.
