ACEs Storytelling Video Featuring SCOE’s Mishale Ballinger: Learning with ACEs, An Educator's Story

The work Sonoma County Office of Education (SCOE) has been doing in collaboration with Sonoma Connect | Sonoma Unidos to support professional development for school staff with trauma-informed practices recently captured the attention of ACEs Aware. Take a look at the storytelling video that illustrates how ACEs science and trauma-informed practices are being applied in Sonoma County schools to help students and educators!

First 5 Sonoma County Implementation Work Group Invitation


In June, First 5 Sonoma County completed a strategic plan to better serve children and families through a coordinated home visiting system. We are now starting to execute the plan. 


First 5 invites you and families in your network to join a Work Group that will lead implementation of the strategies in one of the two priority areas: 


  • Priority 1: Expanding culturally relevant services to underserved areas with home visiting providers who represent the communities they serve. 
  • Priority 2: Increasing the home visiting system's capacity and efficiency. 


The Work Group will meet online each month, from August or September 2022 and through June 2023. We will schedule the meetings in collaboration with the group members. Each Work Group will have 9 members. Each home visitor and parent will receive a $1,250 stipend for their participation in the Work Group. 


If you would like to join the group, please complete this interest form by Wednesday, August 24th. Participation open to leadership/management of home-based programs, home visitors or other service providers delivering services in families’ homes, and parents/family members who participate in home-based services.

This month, the Sonoma Connect | Sonoma Unidos Action Teams will be working on the agenda items below:


ACEs Clinical Action Team 

Thursday, August 4, 2022, 2:00pm-3:00pm 

The ACEs Clinical AT will be discussing the First Five Pathway to Income Equality opportunity as well as giving a brief update on the Resource Connection Network platform. We will also be discussing SDOH specifically related to the possibility of integrating PRAPARE screenings into this group's focus moving forward.


Community Accountability Action Team 

Wednesday, August 10, 2022, 3:00pm-4:30pm 

Join us as we discuss what a relational culture means and devise ways to create a culture grounded in these principles, rather than white supremacy cultural norms. We will also discuss best practices for gathering demographic data.


Resource Connection Network Action Team  

Monday, August 29, 2022, 1:00pm-2:30pm 

We will be sending an updated calendar invite as this group transitions from the Resource Connection Network Action Team into a Data Governance Committee. Join the next meeting to help us review the language of data sharing consent forms and other information needed for the implementation of the health and social integration platform.


Our Action Team meetings offer simultaneous interpretation in Spanish.

 Health and Social Integration Platform Q&A

How did we select NinePatch?


The Resource Connection Network (RCN) Action Team convened with 79 participants from 45 different organizations to identify desired criteria for the technology selection. Based on the criteria, Redwood Community Health Coalition (RCHC) worked with consultants Intrepid Ascent to develop a Request for Proposal (RFP) to different technology vendors. The RCN team then organized five ad-hoc meetings where the technology vendors had the opportunity to present their response to the RFP. Based on the vendors’ response to the RFP, community stakeholders were then asked to score each vendor. NinePatch scored the highest based on their ability to respond to the desired functions and was recommended for selection. The Community Leadership Board of Sonoma Connect | Sonoma Unidos then voted to ratify the community’s recommendation and select NinePatch. 


Why is it needed?


This platform will help connect participants to resources available in their communities that service providers wouldn’t otherwise be aware of. In Sonoma County there are many existing data systems, and this results in duplication of data entry and an uncoordinated approach to reducing disparities and inequity. Community stakeholders decided that we needed a tool that could support cross-sector referral and workflows to improve resource coordination and implement a unified approach to reducing disparities and inequity in our community.   


Why did we select this platform?


Our Community Stakeholders selected NinePatch because it ranked high on desired functions which stakeholders decided were a priority. Among the desired functions the most high-ranking ones were the ability to have a system that had interoperability capacity and the ability to track the outcome of a referral through a closed-loop referral function. 


What is interoperability?


Because there are many existing data systems, one of the desired functions ranked high by community stakeholders was interoperability. Interoperability is the ability for the platform to digitally exchange information with existing data systems. The goal is to work with existing systems as much as possible, and to increase coordination among community partners. 

What are closed-loop referrals?



We love this description from Connecting Providers with Technology to Improve Patient Care (CRISP) on closed-loop referrals: “Closed-loop referrals provide a means for healthcare professionals to send patient information to a community-based organization (CBO) to help address a patient’s needs that are typically better served outside of clinical workflows.

 A CBO can provide an array of different support programs within the community, including services that address a patient's social needs or address underlying causes of poor health outcomes with the goal of positively impacting the patient’s overall health outcome(s). The CBO can then provide feedback on the outcome of that referral back to the referring individual/entity.” 

How will this platform be implemented?



This platform will be co-created with input and feedback from the community and will be implemented through the following steps:

1.    Community engagement to drive platform goals, priorities, technical criteria

2.    Platform chosen and acquired

3.    Build system and workflows with community input (WE ARE HERE)

4.    Initial implementation and bug fixes with pilot cohort

5.    Outreach and Training

6.    Expanded implementation with additional partners

7.    Build out and implementation of additional functionality

La Luz Center ESL Classes


La Luz offers 5 levels of ESL classes mornings and evenings. Classes on Monday and Wednesday will be offered via Zoom and Tuesdays and Thursdays in-person at La Luz Center. Enrollment fee is $20 and reading material $35 (scholarships are available, if needed). Classes will be offered from August 22-December 15.
