Person-Centered Service Planning

Applicable to: Mercy Care Long Term Care and Mercy Care DD
Person-Centered Service Planning supports ALTCS members in planning and creating the life they want through services and supports. AHCCCS has evaluated and revised person-centered service planning standards, practices and forms. This will improve support for community integration so members have full access to the benefits of community living. This initiative is part of the State’s plan to comply with the new Home and Community Based Settings Rules and will go into effect on June 1, 2021.

The goal is to ensure a standardized method for reviewing and documenting discussions with members during review and service planning meetings. The idea being that it’s not about the form itself, rather it’s about documenting the discussion.

The initial meeting for each member using the new tool will take longer than it did previously. This is in part because it is new to both staff and members but also due to the conversational approach to the review. Mercy Care will be training the Case Management (CM) staff to use best judgement when scheduling with members and working with facility staff. The first review may be broken into two sessions on dates close to each other. Mercy Care would like facility staff to know they can suggest this at the time they are contacted if it helps address their own staffing and virtual equipment needs.

Additionally, Mercy Care CM Staff will be working proactively to hold reviews with members and their support network to minimize short notice requests for information and input. Member reviews will continue to occur at 90 or 180 day intervals based on placement setting. Each member’s support network will be identified by them in discussion with their case manager. Invitations for input and collaboration will be extended with provider/facility responses and feedback collected before, during or shortly after the review meeting as best fits their schedule. Facility feedback can be provided telephonically, via email and in person (when we return to face to face visits).
